| Objective:To observe the effect of stroke promoting moxibustion on sleep disorders after ischemic stroke,to explore its effect on neurological function recovery,sleep improvement and psychological state,and to analyze its mechanism of action,and to explore the clinical value of stroke promoting moxibustion on sleep disorders after ischemic stroke.Methods:60 patients with sleep disorders after ischemic stroke were allocated into treatment group and control group in randomization.Both groups were given routine treatment to control blood pressure,blood sugar,blood lipid and symptomatic treatment,control basic diseases and prevent complications.The control group was given routine acupuncture treatment,and the treatment group was treated with stroke promoting moxibustion on the basis of control group.All treated 5 times a week,once a day,no treatment on Saturday and Sunday.A period of treatment includes 10 times of treatment,and two periods in the aggregation of treatment.We conducted follow-up survey after 3 months.The scores of NIHSS,PSQI,HAMA and HAMD-24 were recorded before and after both treatments,the differences in neurological function recovery,sleep improvement and psychological status before and after both treatments were compared.3 months later,we evaluated the prognosis and recurrence rate of sleep disorders after ischemic stroke.Spss21.0 was used to analyze all the clinical data.Results:1.There was no statistical significance in gender,age,NIHSS、PSQI、HAMA、HAND-24 scores before treatment(P>0.05).2.After 4 weeks of treatment,the difference of NIHSS score between the two groups before and after treatment was statistically significant(P<0.01),the difference between the two groups after treatment was statistically significant(P<0.01).3.After 4 weeks of treatment,it was found that the differences of sleep quality,time to fall asleep,total sleep time,sleep efficiency,sleep disorders,sleeping drugs,daytime dysfunction scores and PSQI total scores between the two groups were statistically significant(P<0.01)by observing the PSQI scores and total scores of the two groups before and after treatment.There were statistically significant differences between the two groups after treatment in sleep quality,time to fall asleep,total sleep time,sleep efficiency,sleep disorders,sleeping drugs scores and PSQI total score(P<0.01),but no significant differences in daytime dysfunction(P>0.05).The gross effective rate of the treatment group and the control group in insomnia after stroke was 90.0%and 73.3%,respectively,the difference was statistically significant(P<0.05).4.After 4 weeks of treatment,the difference of HAMA and HAMD-24 scores between the two groups was statistically significant(P<0.01),and the difference between the two groups after treatment was statistically significant(P<0.01).The total effective rates of the treatment group and the control group were 96.67%and 56.67%respectively,and 86.67%and 63.33%respectively(P<0.01).5.Follow-up of post-stroke sleep disorders prognosis and analysis of recurrence rate:the difference was statistically significant(P<0.05)between the treatment group after 3 months’follow-up and the treatment group after 4 weeks,and the difference was not statistically significant(P>0.05)in the control group.The recurrence rate was 6.67%in the treatment group and 16.67%in the control group after 3 months’follow-up(P<0.05).6.Safety and adverse reactions:there were no unsafe events during the treatment.Some patients suffered from headache,nausea,drowsiness and other adverse reactions due to drug factors.The incidence of adverse reactions in the treatment group was 6.67%,while that in the control group was 16.67%(P<0.05).Stroke promoting moxibustion can reduce the incidence of adverse reactions.Conclusion:1.Stroke promoting moxibustion has a good clinical effect on sleep disorders after ischemic stroke.It can significantly improve neurological function,improve sleep quality,shorten sleep time,increase total sleep time,improve sleep efficiency,reduce sleep disorders,reduce the use of hypnotics,improve anxiety and depression,which has obvious advantages over acupuncture control group.2.Stroke promoting moxibustion should be widely used in clinical practice because of its good prognosis,low recurrence rate and high clinical safety. |
