| Part 1 Clinical Value of TBSRTC and TI-RADS in the Differential Diagnosis of Thyroid Nodules.Aim:To compare the value of the Thyroid Imaging Reporting and Data System(TI-RADS)introduced by the American College of Radiology(ACR)and The Bethesda System for Reporting Thyroid Cytopathology(TBSRTC)in the differential diagnosis of thyroid nodules,and to explore the diagnostic value of their combination.Methods:A total of 494 patients with 545 thyroid nodules were enrolled.All thyroid nodules were evaluated with pre-operative ultrasound examination and ultrasound-guided fine-needle aspiration cytology,and their final diagnosis were confirmed by postoperative pathohistology.Both TBSRTC and TI-RADS were introduced,and the diagnostic efficacy was assessed in their separate and combined use based on the final histology.Result:1.Among 545 thyroid nodules,81 and 464 were confirmed as benign nodules and papillary thyroid carcinoma.The malignancy rate was 85.14%.2.The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of TI-RADS were 97.20%,45.68%,91.11%,74.0%,and 89.54%,respectively.3.The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of TBSRTC were 79.96%,72.84%,94.40%,38.82%,and 78.90%,respectively.4.After TBSRTC was used in combined with TI-RADS,the sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy were 99.18,71.43%,97.30%,89.29%,and 96.74%,respectively.5.Among 285 nodules with indeterminate cytology,33 and 252 were confirmed as benign nodules and papillary thyroid carcinoma.The malignancy rate was 88.4%.For thyroid nodules with indeterminate cytology,the sensitivity,specificity,positive predictive value and negative predictive value of TI-RADS were 98.02%?42.42%?92.86%?73.68%?91.58%,respectively.Conclusion:The diagnostic value of TBSRTC in the differentiation of benign and malignant thyroid nodules is better than TI-RADS,and the combination of both reaches the best diagnostic efficiency.The use of ACR TI-RADS should be considered and encouraged for thyroid nodules with indeteminate cytology.Part2.Effect of Banxia Houpu Decoction on Globus Symptoms after the surgery of thyroid cancerAim:To investigate the clinical effect of Banxia Houpu Decoction on pharyngeal foreign body sensation after the surgery of thyroid cancer.Methods:A total of 62 patients with thyroid cancer undergoing thyroidectomy in Jiangsu Province Hospital on Integration of Chinese and Western Medicine from September 2019 to December 2019 were randomly divided into 2 groups:intervention group and control group.Patients in control group received routine neck exercise.Patients in intervention group were treated with Banxia Houpu decoction while receiving routine neck exercise.The scores of The Foreign Body Sensation in the Throat Score(FBST)and Traditional Chinese Medicine(TCM)symptom score scale before and after treatment were also observed.The SPSS20.0 was used in data analysis to evaluate the clinical efficacy of Banxia Houpu Decoction.Results:1.Comparison of the FBST:The comparison between two groups of pharyngeal paresthesia scores before and after treatment(P<0.05)showed that pharyngeal paresthesia symptoms were significantly improved;The comparison between two groups of the difference of the scores after treatment(P<0.05)showed that the improvement on the symptoms of pharyngeal foreign body sensation in the treatment group was more significant than that in the control group.2.Comparison of the TCM symptom scores:The comparison between two groups of TCM scores before and after treatment(P<0.05)showed that the symptoms of TCM were significantly improved in both groups.The comparison between two groups of the difference of the scores after treatment(P<0.05)showed that the total symptoms of TCM in the treatment group was more significantly improved than that in the control group.The comparison between two groups of the difference of the individual scores after treatment showed that the effect of the treatment group was better than that of the control group in improving the abnormal sensation of throat and expectorate,the treatment group and the control group were equivalent in improving sighing and chest tightness.3.Comparison of overall efficacy:The total effective rates of the treatment group and control group were 83.33 and 36.67%,respectively.The overall efficacy of the treatment group was significantly satisfactory than that of the control group.Conclusion:Banxia Houpu Decoction improves pharyngeal foreign body sensation and TCM symptoms in patients with thyroid cancer after surgery. |
