Objective:To explore the mechanism of improving gastrointestinal tumor patients' symptoms with deficiency of qi and Yin after the second chemotherapy treating by yiqiyangyin prescription.Methods:According to the double blind experimental principle,100 gastrointestinal tumor patients who after twice chemotherapy collected from Zhangjiagang hospital of Chinese Medicine and Changshu No.1 People's Hospital,both in Suzhou,Jiangsu province during August 2018 to December 2019 were randomly divided into three groups:high-dose group,low-dose group and placebo group with 34,32 and 34 patients.The high-dose group received two bottles of oral liquid,the low-dose group received one bottle of oral liquid and one bottle of placebo,and the placebo group received two bottles of placebo for 30 days,twice a day.Comparing clinical indexes and intestinal microbiota of patients between before and after taking yiqiyangyin prescription.At the same time,30 volunteers were selected to detect intestinal flora as a healthy group.Results:(1)the prescription had no significant effects on indexes related to liver,kidney,bone marrow inhibition and cancer.(2)in the high-dose group,CD3+(t=-2.1304,P=0.0409)and CD4+/CD8+(t=-2.0489,P=0.0499)were significantly increased,in the low-dose group,CD4+/CD8+(t=-2.3840,P=0.0234)was significantly increased;(3)comparing before and after clinical symptoms of the three groups,the score of the high-dose group decreased by 41.49%,the score of the low-dose group decreased by 33.40%,and the score of the placebo group decreased by 23.25%;(4)compared with normal people,the intestinal flora structure of the patients showed significant differences in Firmicutes(t=-3,8129,P=0.0002),Proteobacteria(t=5.1129,P=0.000001)and Actinomycetes(t=2.8206,P=0.0058);(5)Using heteroscedasticity T test,found out that patients were significantly lower than normal people in sobs,shannon,Simpson,Chao1 and ace indexes,beta diversity:the intestinal microbiota structure of patients was significantly different from normal people' the intestinal microbiota structure;(6)at the phylum level,the Proteobacteria of the high-dose group(t=2.2154,P=0.0354)were significantly reduced.At the genus level,the genus Escherichia-Shigella(t=2.0916,P=0.0477)significantly decreased in the high-dose group,and significantly increased in the placebo group(t=-2.0724,P=0.0476);(7)dividing each group with the normal people's proteobacteria abundance(4.92±6.98)of 12%,and it was found that in high dose group with high proteobacteria,the proteobacteria(t=2.3040,P=0.0359)and Escherichia-Shigella(t=2.2831,P=0.0455)decreased significantly.In the placebo group with low proteobacteria,proteobacteria(t=-2.4379,P=0.0237)and shigella esch(t=-2.1616,P=0.0436)increased significantly.Conclusion:One of the mechanisms of improving qi and Yin deficiency syndrome in gastrointestinal tumor patients after second chemotherapy is improving immune function and intestinal microenvironment,inhibiting excessive proliferation of proteobacteria and Shigella.The high dose has better effects.
