Objective:Eosinophil counts have been shown in previous studies to significantly affect mortality in critically ill patients and can be used to assess outcomes in icu patients.However,the relationship between eosinophil and the mechanism of acute cerebral apoplexy is still unclear,and there are few clinical studies on it,which is alsoa hot topics in recent years.In this study,the general clinical characteristics and prognosis of patients with acute stroke at different eosinophil levels were compared and analyzed to explore the correlation between different eosinophil levels and acute stroke conditions and short-term prognosis.Methods:To 2018.09 ~ 2019.12 in Wannan medical college first affiliated hospital neurology ward of 303 patients with acute stroke patients who were hospitalized in the hospital the next morning on an empty stomach pumping elbow venous blood,use Wannan medical college first affiliated hospital clinical laboratory equipment measuring its related biochemical and conventional index,and press the eosinophil percentage is less than 0.5% of the total number of white blood cells into eosinophils reduced(Eosino-)group and normal eosinophils(Eosino)group,and within 30 minutes after admission and discharge on the day of the completion of The National Institutes of Health Stroke Scale(NIH stroke scale,NIHSS),in 3 months after discharge clinics or telephone follow-up completed modified Rankin scale(mRS)score,for evaluating the illness severity and short-term prognosis index,and further affect the severity of thepatients with acute stroke and short-term prognosis of risk factors for Logistic regression analysis.Results:The percentage of eosinophil in the group with poor prognosis for acute ischemic and hemorrhagic stroke was significantly lower than that of the group with good prognosis(P <0.001),with age(P=0.048,OR=1.031,95%ci =1.000~1.062),leukocyte(P=0.027,OR=1.170,95%ci =1.018~1.344),percentage of neutrophils(P=0.032,OR=1.052,95%ci =1.004~1.101),and eosinophil(P=0.012,OR=2.771,95%ci =1.252~6.136),infection(P=0.006,OR=2.825,95%ci =1.348~5.920)and admission NIHSS score(P=0.001,OR=2.451,95%ci =1.828~3.287)were the main risk factors for poor short-term prognosis in patients with acute stroke.Compared with acute stroke Eosino group,patients in the Eosino-group were significantly more likely to have advanced age and history of atrial fibrillation,and the risk of infection was also significantly increased,the fasting blood glucose,WBC,neutrophil percentage,systolic blood pressure,heart rate,length of hospital stay,hospital admission NIHSS score and dischargeNIHSS score,and3 months after discharge mRS score significantly higher than that of Eosino group(P < 0.05).In patients with acute ischemic stroke,most patients with cardiogenic stroke showed eosinophilseverely(P<0.05),while most patients with small-vessel occlusive stroke showed normal eosinophil levels(P<0.05).Conclusion:Patients with acute stroke accompanied by eosinophilseverely showed more severe neurological deficits and poor short-term prognosis(P<0.05),significantly increased risk of infection during the course of the disease and prolonged hospital stay.Although increased neutrophils(P=0.032,OR=1.052,95%ci =1.004~1.101)were also an adverse factor affecting the prognosis of acute stroke,the effect of eosinophil on the mechanism of acute stroke was not affected.Eosinophilseverely(P=0.012,OR=2.771,95%ci =1.252~6.136)is an independent risk factor for poor short-term prognosis of acute stroke,which can be used to effectively evaluate the prognosis of patients and provide objective basis for clinical treatment.
