Objective To screen out the core nursing outcomes of inpatients with T2 DM based on the third edition of Nursing Outcomes Classification,and to use Delphi method to revise the core screening outcomes and indicators of primary screening to make them more consistent with national conditions,cultural background and Language understanding within China.The aim is to extract the core nursing outcomes that embody the essence of the nursing practice of diabetes specialists,clarify the core and key content of nursing work for inpatients with T2 DM,and provide a standardized tool for comprehensive and efficient continuous assessment of patients' condition and scientific measurement of the effectiveness of nursing measures.Method 1.Review literature to select nursing diagnosis related to T2 DM,and then conduct a status survey in the clinic to analyze the frequency of nursing diagnosis of inpatients with T2 DM to determine their common nursing diagnosis.Check the NANDA-NOC link in NOC to select the relevant nursing outcomes of hospitalized patients with T2 DM,and a questionnaire survey of the importance of nursing outcomes among 76 clinical nurses was used to screen the core nursing outcomes of hospitalized patients with T2 DM.2.Used the Delphi method to conduct two rounds of consultation on the core screening outcomes of the preliminary screening.Then according to the experts' recommendations as well as group discussions,the revision of the core nursing outcomes of patients with T2 DM is conducted.Results 1.Through literature review,there were 16 nursing diagnosis of T2 DM.According to 273 patients' existing or potential health problems,the frequency of each nursing diagnosis is counted.There are 6 common nursing diagnoses identified,which are in order of Deficient of knowledge,Risk for infection,Risk for peripheral neurovascular dysfunction,Risk for falling,Imbalanced nutrition and Anxiety.2.According to the screening condition of core nursing outco me "mean importance ? 4.0",7 core nursing outcomes of T2 DM hosp italized patients were preliminarily screened,which were: Blood glucose level,Weight control,Anxiety self-control,Fall prevent ion behavior,Knowledge:Diabetes Management,Tissue Perfusion: Low er extremity,Tissue Integrity: Skin and Mucous membranes.3.23 experts completed two rounds of consultation and their authority coefficient was 0.89.The recovery rates of the two rounds of expert consultation tables were 92% and 100%,and the ranges of Kendall's W values of the two rounds of expert consultations were 0.115 ~ 0.206,0.45 ~0.226,And the Kendall's W value in the second round was significantly higher than that in the first round,which was statistically significant(P < 0.05).After two rounds of expert consultation,the average value of the importance of 8 nursing outcomes ranges from 4.40~4.88,and the coefficient of variation fluctuates within 0.08~0.21;the average value of the importance of 122 outcome indicators ranges from 4.14~4.80,which varies coefficient range fluctuates between 0.10~0.244.According to the suggestions of experts,added the nursing outcome "Diabetes self-management",modified the description of 2 nursing outcomes,deleted 47 outcome indicators,added 2 new outcome indicators,merged 5 outcome indicators,modified 12 outcome indicator names,and finally formed 8 core nursing outcomes for inpatients with type 2 diabetes with a total of 122 outcome indicators.Among them,there are 3 items in the field of physical health-blood glucose level,tissue perfusion: lower extremity,tissue integrity: skin and mucous membranes,a total of 33 indicators.4 items in the field of health knowledge and behaviorweight control,fall prevention behavior,knowledge: Diabetes management,diabetes self-management,a total of 76 indicators.Derived from 1 in the field of psychosocial health-anxiety selfcontrol,including 13 indicators.Conclusion With reasonable and scientific screening process and methods,the core nursing outcomes of screening can reflect the essence of nursing practice in diabetes;clarify the core content of nursing work.The revised results produced by Delphi method are reliable and provide a standardized tool for efficient and comprehensive assessment of the patient's condition and scientific measurement of the effectiveness of nursing measures,and thereby improving the quality of diabetes specialist care and reflecting the value of nursing staff.In the future,it is necessary to further test its reliability and validity in clinical practice,linking nursing outcome classification(NOC)with nursing diagnosis(NANDA-I)and nursing measure classification(NIC)to form an NNN link to provide comprehensive standardized nursing terms for clinical nursing staff.
