ObjectiveTo observe the effect of desflurane and sevoflurane on brain oxygen metabolism and postoperative recovery quality in robot assisted radical cystectomy.MethodsSixty elderly patients undergoing robot-assisted radical cystectomy for bladder cancer were randomly divided into two groups by random number table method:Group D(the maintenance anesthesia of inhalation was performed by 1MAC Des combined with 0.2-0.3ug/kg/min Remifentanil)?Group S(the maintenance anesthesia of inhalation was performed by 1MAC Sev combined with 0.2-0.3ug/kg/min Remifentanil).The two groups were mechanically ventilated after general anesthesia induction,then maintain PaCO2 at 35-45mmHg,BIS values between 40-60 and nasopharyngeal temperature between 36?-37?.Patients' HR,CO,Hba,SaO2,PaO2,Glua,Laca,Hbjv,SjvO2,PjvO2,Glujv,Lacjv were observed and recorded at 3 min after induction of anesthesia(T0),after inducing 30 min Trendelenburg position(T1),after inducing 90 min Trendelenburg position(T2)and after desufflation in a supine position 20 min(T3).Then calculated the Da-jO2,CERO2,CGU and CLP.Then Blood oss,operation time,T position time,opening eyes time,hand-squeezing time,extubation time,recovery time of orientation and PACU time were recorded.ResultsThere was no significant difference in age,height,weight and preoperative hemoglobin between the two groups(P>0.05).There was no significant difference in blood loss,T position time and PACU time between the two groups(P>0.05).Compared with group S,HR?Hba?Hbjv?CGU?CLP had no significant difference in group D at each time point(P>0.05).Compared with group S,CECO2 in group D decreased significantly at T1 and T2(P<0.05).Comepared with group S,the opening eyes time,hand-squeezing time,extubation time and recovery time of orientation decreased significantly in group D(P<0.05).Comparing within the group:the increase of CO is more obvious at T1 and T2 than T0(P<0.05);the decrease of CERO2 is more obvious at T2 than T0(P<0.05).Conclusion1.Both desflurane and sevoflurane can reduce the rate of brain oxygen metabolism in robot assisted radical operation of bladder cancer in elderly patients,and the effect of desflurane is more significant.2.In robot assisted radical operation of bladder cancer in elderly patients,desflurane can shorten the time of recovery and extubation which is more beneficial to functional recovery.
