Objective To investigate the status of depression and anxiety with psychological scale in breast cancer patients,and find out its influences.Further explore the relationship between breast conserving surgery and radical mastectomy with depression and anxiety in breast cancer patients,so as to provide a scientific basis for patients with breast cancer surgery to establish a diagnosis and treatment model of body and mind treatment.Methods 80 patients who were clinically diagnosed with breast cancer and underwent surgical treatment in Tongde hospital of Zhejiang province or Zhejiang cancer hospital from January 2018 to July 2019 were enrolled.Demographic data,such as the age and degree of education,as well as clinicopathological data,such as surgical methods and whether to receive postoperative adjuvant therapy,were collected and recorded,and the patients with benign breast tumor diagnosed by pathology were excluded.Then divide the patients into breast-conserving group and mastectomy group according to the surgical method,and assess the patient's psychological status before surgery,1 week after surgery and 6 months after surgery with Self-Rating Depression Scale and Self-Rating Anxiety Scale.Finally,the results were analyzed by statistical methods.Results In this study,among the 80 patients were enrolled,11 were benign tumor,1 was lost to follow-up,and 5 were incomplete in the questionnaire,with the lost to follow-up rate of 1.44%and the unqualified rate of 7.35%.Finally,63 patients were identified as the research object.The depression scores of the 63 patients before surgery,1 week after surgery and 6 months after surgery were(48.05±5.030),(55.63±7.153),(53.44±4.809),and the anxiety scores were(53.67±5.433),(59.70±5.399),(53.35±4.337).The average scores of depression and anxiety in the three periods were higher than the domestic norm,and the scores at 1 week after surgery were the highest.The analysis results of the factors influencing psychological status showed that the factors affecting depression and anxiety in patients with breast cancer surgery included degree of education,menstrual status and surgical method(P<0.05).According to different surgical methods,the depression scores of patients in the breast-conserving group at three times were(47.10±5.628),(50.60±5.443),(52.05±3.734);the anxiety scores were(51.95±5.266),(63.50±4.371),(54.70±4.169),and the depression scores in the mastectomy group were(48.49±4.733),(57.98±6.667),(54.09±5.145);the anxiety scores were(54.47±5.382),(57.93±4.930),(52.72±4.317).The depression score in the mastectomy group was significantly higher than that of patients in the breast-conserving group,while the anxiety score in the breast-conserving group was significantly higher than that of patients in the mastectomy group at 1 week after surgery(P<0.05).However,there was no statistically significant difference in the scores of depression and anxiety between the two groups at 6 months after surgery(P<0.05).Conclusion The majority of patients with breast cancer surgery have depression and anxiety,which were the most serious at 1 week after surgery and improved at 6 months after surgery.Degree of education and menstrual status were independent risk factors for depression and anxiety in patients with breast cancer surgery.The surgical method had a direct impact on depression and anxiety,patients who underwent radical mastectomy were more likely to appear depression,while those who underwent breast conserving surgery were more likely to appear anxiety.It provides a scientific basis for the selection of surgical methods and the corresponding psychological intervention in breast cancer patients.
