Clinical Study Of Lymph Node Dissection In Radical Thyroidectomy With Complete Endoscopic And Open Surgery

Objective: To investigate the feasibility and clinical value of complete endoscopic dissection of cervical lymph nodes in radical resection of differentiated thyroid cancer.Methods: The data of 60 patients with thyroid papillary carcinoma admitted from June 2016 to June 2018 were collected,including 28 patients in the endoscopy group and32 patients in the open group.The endoscopic group received radical thyroidectomy through bilateral approaches to the areola of the thorax and the areola of the breast,while the open group received radical thyroidectomy along the neck collar,compareing the treatment efficacy of the two surgical approaches and the incidence of postoperative complications.Results: The number of lymph nodes cleaned in the two groups(II,III,IV,VI),the proportion of cancer metastasis,the size of the tumor,common postoperative complications(numbness of the hands and feet,hoarseness,drinking water,cough,lymphatic leakage,subcutaneous There was no statistical difference in the observation index P>0.05.Patients in the endoscopic group had more postoperative hospital stays,24 hours postoperative drainage,and more operative time than the open group.There were statistical differences between the two groups(P<0.05).There was a statistical difference in the age and intraoperative blood loss of the laparoscopic group compared with the open group(P<0.05)Conclusion: Complete endoscopic radical thyroidectomy has the same effect as open surgery for cervical lymph node dissection.Both have the same therapeutic effect in terms of therapeutic efficacy and postoperative complications.Both surgical methods have their own advantages and disadvantages.However,the endoscopic surgical incision is more concealed,and its aesthetic effect is more intuitive than the open group.
