Objective:objective to compare the surgical trauma,postoperative rehabilitation and short-term effects of total thoracoscopy and traditional median thoracotomy in the treatment of mitral valve disease.To further investigate the reliability,safety and efficacy of total thoracoscopic mitral valve replacement in the treatment of mitral valve disease.Methods the clinical data of 92 patients with simple mitral valve replacement performed in the Department of Cardiac surgery,the first affiliated Hospital of Xinjiang Medical University from January 2015 to May 2019 were studied retrospectively.According to the mode of operation,the patients were divided into two groups: complete thoracoscopy group(n = 42),male 17 cases,female 25 cases,age(42.93 ±10.93)years old,no intraoperative conversion to thoracotomy;There were 50 patients in the traditional median thoracotomy group,including 23 males and 27 females,aged(43.92 ±6.58)years.The general data before operation,the clinical indexes related to postoperative prognosis and the short-term curative effect of the two groups were collected and compared.Results 92 patients underwent operation successfully.The complete thoracoscopy group was superior to the median thoracotomy group in erythrocyte infusion,plasma infusion,incision length and postoperative thoracic drainage,and the difference was statistically significant.However,the operation time,cardiopulmonary bypass time and aortic clamping time in the thoracoscopy group were significantly longer than those in the traditional thoracotomy group,and the difference was statistically significant.There was no significant difference in operative complications,postoperative ICU stay days,postoperative hospitalization days,total cost,left atrial size,left ventricular size,EF,FS value and so on.Conclusion compared with the traditional median thoracotomy,total thoracoscopic surgery can significantly reduce the use of blood products and less trauma,and there is no significantdifference in postoperative follow-up indexes between the two groups.Total thoracoscopic mitral valve replacement is safe and effective in the treatment of mitral valve disease,which is worthy of further clinical study.
