Analysis Of Clinical Characteristics Of Diabetic With Fastingblood Glucose Standard Under Degludec In Insulin Treatment

Objective: The clinical characteristics of diabetic patients with fasting glucose reaching the standard after 12 weeks of initial degludec insulin treatment were retrospectively analyzed to provide reference for the optimal selection of initial degludec insulin treatment.Methods:Data were collected from 148 patients with type 2 diabetes who were admitted to the first affiliated hospital of xinjiang medical university from January2018 to December 2019 and reached fastingblood glucose standard after 12 weeks of degludec insulin treatment due to poor blood glucose control.The clinical characteristics of these patients were summarized and analyzed.Results: There are 5 main reasons for initiation of insulin therapy: untreated,oral drugs therapy,premixed insulin therapy,basal insulin therapy and other conditions.The proportions of various causes were as follows:basic insulin treatment in 71 cases,and other conditions in 3 cases.Various reasons starting degludec insulin treatment in age,BMI,course,glycosylated hemoglobin,before starting degludec insulin treatment of fasting blood glucose,blood sugar 2 hours after meal,the frequency of hypoglycaemia,insulin,blood lipids,hypertension and high blood pressure of different grading fine all have their own characteristics.In the patients of the basic insulin treatment group,the insulin dose of degludec(0.362±0.161)U/kg.d and the original insulin(0.417 ± 0.191)U/kg.d at the beginning of insulin treatment could be almost isodose conversion.When fasting blood glucose reached the standard,the average insulin dose(0.313±0.149)U/kg.d was significantly lower than that of the original regimen(0.417±0.191)U/kg.d.The average insulin dose(0.358±0.063)U/kg.d in the untreated group was the largest reduction compared with the insulin dose(0.475±0.277)U/kg.d in the initial deludec insulin treatment.After treatment with degludec insulin,the incidence ofhypoglycemia in each group was significantly reduced,and that of the premixed insulin group was the most significant.Conclusion: Degludec insulin is suitable for patients with type 2 diabetes whose fasting blood glucose level is below the standard,and can reduce the frequency of hypoglycemia,especially for diabetic patients whose fasting blood glucose level is below the standard with more frequency of hypoglycemia treated by premixed insulin.The dose of insulin used in each group was significantly lower when fasting blood glucose reached the standard than when insulin was used at the beginning of degludec.
