Objective: To investigate the correlation between serum cystatin C and diabetic retinopathy,and to find simple and reliable serological indicators for early screening and diagnosis of diabetic retinopathy.Methods: A total of 704 patients with type 2 diabetes hospitalized in Endocrinology department from January to December 2018 were enrolled.General clinical data,laboratory tests,and fundus examination data were collected.Statistical analysis was performed using SPSS 23.0 statistical software.The test level was set as0.05,and P<0.05 was considered statistically significant.Results:(1)There were 427 males(60.7%),the average age was(55.98 ± 11.00)years,the average duration of diabetes was(8.22 ± 6.47)years,and the total detection rate of DR was 57.4%.As the Cys C quartile increased,the average age,duration of diabetes,and DR detection rate gradually increased,and the differences were statistically significant(P<0.05).(2)The detection rates of NPDR and PDR were 45.3% and 12.1%,respectively.The detection rates,whether in the non-proliferative or proliferative phase,increases with the Cys C quartile.The differences between NDR group and NPDR group,as well as PDR group were statistically significant(P<0.05).(3)With the aggravation of DR degree,the average duration of diabetes and level of serum Cys C in the three groups gradually increased.The average duration of diabetes in PDR group was significantly increased compared with that in NDR group and NPDR group,and the difference was statistically significant(P<0.05).Compared with the NDR group,serum Cys C level in the NPDR and PDR groups was significantly increased,and the difference was statistically significant(P< 0.05).(4)Pearson correlation analysis showed that serum Cys C was positively correlated with age,duration of diabetes,hypertension,DR,Scr,and SUA,and negatively correlated with Ccr(P<0.001).(5)Binary logistic regression analysis showed that serum Cys C is a risk factor for DR(P<0.05).The higher the serum Cys C level,the higher the risk of DR.(6)ROC curve analysis showed that the area under the curve(AUC)of serum Cys C predicting DR was 0.584(P<0.05),the optimal cutoff value was 0.69 mg/L,the sensitivity was 51.5%,and the specificity was63.0%.Conclusions:In this study,the total DR detection rate of T2 DM patients was 57.4%,of which the PDR detection rate was 12.1%.As the Cys C quartile increased,the detection rate of DR gradually increased.The higher the degree of DR,the higher the level of serum Cys C.Serum Cys C is correlated with DR,and is a risk factor of DR,and can be used as a serological indicator to predict DR.
