Objective:To assess the relevant clinical factors of patients with HPV persistence and their HPV clearing time,Finding the Predictors of HPV persistence after cervical conization.Methods:1.Choosing 210 patients with hight-grade cervical intraepithelial that was proved by biopsy for retrospective analysis.all of them was detected by HPV genotyping and/or HPV HC2?TCT at 3,6,12,18,24 months after operation.choosing 30 patients with HPV persistent infection as experimental group,regarding the remain 180 patients that without HPV infection as control group.then analyse of related factors(such as age?menopausal status ? HPV genotyping,HPV HC2 and TCT preoperative,The cone of cutting depth,pathological grading,state of Surgical margin and so on)For HPV persistent infection and its clearing time after cervical conization.2.Choosing 30 patients with HPV persistent infection as experimental group,regard ing the corresponding 30 patients without infection as control group.choosing 20 cervical cancer and 20 Normal cervical(the patients with total hysterectomy for hysteromyoma)for correlation analysis.using their wax block after conization for immunohistochemical.Contrasting the diffrence expression of B7H4 between them.Results:1.30 patients(14.3%)was found with HPV persistence and 8(3.8%)patients was found with persistence/recurrence of cervical intraepithelial neoplasia after cervical conization.Binary logistic analysis showing that the HPV persistenc after cervical conization was closely related by the state of surgical margin,the cervical glands involvement,HPV genotyping and HPV HC2 befor operation.The result of multiple logistic analysis showing that the independent predictors of HPV persistenc postoperation was HPV genotyping and HPV HC2 befor operation(P<0.05).And that was irrelevant with age?menopausal status?TCT preoperative?The cone of cutting depth?pathological grading?state of Surgical margin and so on(P>0.05).2.The HPV clearing time is longer in patients with hr-HPV 16?18 infection than patients with other type of HPV infection at 3,6,12,18 month after cervical conization.The HPV clearing time is longer in patients when their preoperative HPV HC2?100RLU/CO than that of patients with HPV HC2<100RLU/CO at 3,6,12,18 month after cervical conization.The HPV clearing time is longer in patients with gland involvement than that without gland involvement at 3,12 month after cervical conization.The HPV clearing time is longer in patients with postive surgical margin than that with negative surgical margin at 3 month(P<0.05).3.The positive expression rates of B7H4 is higher in patients with HPV persistence infection than that without HPV infectoon after cervical conization(P<0.05).Conclusion:1.The persistent infection of HPV after cervical conization is relatade to HPV genotyping and HPV HC2(P<0.05).2 The high expression rates of B7H4 is related to HPV Persistence after cervical conization.It would be the indicator of cervical disease.
