Aim of the dissertationAcquired melanocytic nevus is a benign skin neoformation.The phenomenon is an informal partial aggregation formed by melanocytes in the process of transporting from neural crest to epidermis as a result of alloplasia.It is difficult to diagnose acquired melanocytic nevus and early malignant melanoma.The practice of the diagnosis of the two is usually by naked eye observation and histopathological examination.Naked eye observation has a weaker susceptibility for early skin lesions,which may lead to missed diagnosis and misdiagnosis;while adopting histopathological examination blindly is hard to apply biopsy one by one when come across multiple skin lesions,and may lead to metast-asis or recurrence.Thus laying great significance on early and noninvasive diagnostic method.Dermatoscope is capable of examing the skin structure and features of underneath parts of the epidermis,papillary layer of corium and deep dermis which cannot be observed by naked eye without invading the skin.The most important significance is to diagnose benign and malignant melanocytes tumors in an early stage without invading the skin.But current diagnostic criteria have been made based on the research of the dermatoscope in foreign countries.The diagnosis was confirmed by histopathology of the skin,and the characteristics of skin microscopy were retrospectively analyzed.This dissertation aims to analyze the dermoscope characteristics of the acromelic acquired pigmented nevus in the Chinese people's,the frequency of the occurrence of each characteristic pattern.And improve the understanding of clinicians toward this disease in order to reduce misdiagnosis and missed diagnosis,providing right and appropriate treatment for patients.The alternating relationship between the dermoscopy signs and histopathology has also been compared and analyzed.MethodologyRetrospective study is used in this research to analyze 117 lesions of 105 patients in Qianfoshan Hospital from the June of 2015 to the December of 2017 who have been diagnosed acquired melanocytic nevus.Dermoscopy and histopathology inspections have been used for the above-mentioned patients.35 male patients and 70 female patients,aged from 3 to 75,with the average age of 38,have been analyzed in this research.Two dermatologists who were trained and experienced by the relevant dermoscope were analyzed for the morphology of skin lesions.Histopathology has been used as the golden criteria in this research,and the histopathological diagnosis has been conducted by two dermatologists respectively who have no understanding of the skin feature analyzed by dermoscopy.In this research,pathological photos of acquired melanocytic nevus diagnosed by histopathology have been compared with dermoscopy patterns summarized by foreign and domestic scholars to analyze the unique signs of dermoscopy result of acromelic acquired melanocytic nevus on Chinese people.To statistic the characteristic patterns appeared frequently the site of the frequency,and pigment nevus each model is special corresponding relation with the pathological.ResultFrom of 117 acquired melanocytic nevus lesions,there are 45 examples of parallel furrow pattern?38.5%?,which is the most common dermoscopy pattern;28 examples of fibrillar pattern?23.9%?;17 examples of lattice pattern?14.5%?;10 examples of mixed pattern?8.5%?;8 examples of homogeneous pattern?6.0%?;7 examples of globular pattern?6.4%?and 2 examples of reticular pattern?1.7%?.Compared with foreign literature[1],fibrillar pattern frequency difference was statistically significant?u=3.189,P<0.05?,but with the domestic literature[2],there is no statistical differences?u=0.251,P>0.251?,shows the people acra acquired the appearance of pigmented nevus fiber sample pattern rate is higher than whites.The frequency of the remaining modes was basically consistent with the literature at home and abroad,and there was no statistical difference?P>0.05?.In terms of the distribution of dermoscopy pattern in skin lesions,the proportion of the parallel furrow pattern in the palm and the foot was roughly the same,no statistically significant difference?u=0.15,P>0.05?.However,there was statistical significance in the distribution of the friction parts and non-friction parts of the foot?u=2.29,P<0.05?.The fibrillar pattern and lattice pattern are almost evenly distributed in the foot?u=4.54,u=2.67,P<0.05?,and all are located in the area where the foot is prone to friction?u=5.10,u=3.74,P<0.05?.There was no significant statistical significance?P>0.05?in globular pattern,homogeneous pattern,mixed pattern in palm and foot plantar distribution.The correspo-nding pathology of each dermoscopy pattern is not completely,but can be expressed as one or more of the junctional nevus,the compound nevus,and the intradermal nevus.Conclusion1.Compared with foreign literature[1],fibrillar pattern frequency difference was statistically significant,but with the domestic literature[2],there is no statistical differences.It shows the Chinese acromelic acquired the appearance of pigmented nevus fiber sample pattern rate is higher than whites.And parallel furrow pattern,lattice pattern,mixed pattern,homogeneous pattern,globular pattern and reticular pattern were no statistical difference.2.Fibrillar pattern and lattice pattern are almost evenly distributed in the plantar,and they are all located in the area of easy to friction.3.The corresponding pathology of each dermoscopy pattern is not completely,but it can be expressed as one or more of the junctional nevus,the compound nevus and the intradermal nevus.A dermoscopy pattern does not indicate a single nevus type.
