Background and objective Liposarcoma(LPS)is the most common in adult Soft Tissue Sarcoma(STS).LS has many characteristics.First,LPS is painless mass and has retarded growth.So,they usually have large size.Second,they have deep position and most commonly occur in lower extremity,retroperitoneal.The pathological classification of LPS is complex.The main treatment is surgical resection.But postoperative recurrence is very high.Repeated recurrence and distant metastasis are main cause of death.All in all,radiotherapy and chemotherapy have poor efficacy in advanced LPS.And there is lack of standard targeted therapy.This paper reported one case of Apatinib as targeted therapy treatment of advanced pelvic LPS,combined with review of the literature,involving the growing zone,pathology,imaging,clinical manifestations and treatment,to raise awareness of the disease.Methods Report a case of Apatinib as targeted therapy treatment of advanced pelvic LPS.Patient saw doctor on account of "abdominal distension" In September 2014.according to ascites cytology,the initial diagnosis was ovarian carcinoma with lung metastasis.but the disease continued to progress after chemotherapy and intraperitoneal chemotherapy.patient had Pelvic mass puncture biopsy under the guidance of B ultrasound after 3 months later.The pathological diagnosis was pleomorphic liposarcoma(PLS),after all chemotherapy and intraperitoneal chemotherapy,disease was still poorly controlled.Patient had poor general physical condition,it was difficult to tolerate surgery and radiotherapy.Given mesylate apatinib 425 mg Po QD,tumor markers in patients decreased.We contrasted the front whole abdominal CT comparison showed that the mass is reduced,the patient have disease SD.Physical fatigue,anorexia and other symptoms improved,Patient had stable condition for 2 months.The patient died because of disease progression finally.We review LPS pa-tients in CNKI and PUBMED database,and analysis of the main location,clinical manifestation,pathological type,imaging findings,treatment and prognosis,in order to improve understanding of the disease.Conclusion At present,liposarcoma most treated by surgical resection.Most patients with early LPS are lack of obvious symptoms.With the larger and larger size of sarcoma and sarcoma compressing the surrounding tissues and organs,patients have local symptoms.The doctor often missed the best period of operation which resulted in poor prognosis.Poorly differrentiated LPS is more complex.Because it is easy to recurrence and distant metastasis after surgery.Radiotherapy and chemotherapy is not sensitive to advanced LS.And there is lack of standard targeted therapy.This paper reported one case of Apatinib as targeted therapy treatment of advanced pelvic LPS,and achieved satisfactory curative effect and tolerable toxicity which suggest that apatinib may be a new strategy for the treatment of advanced LPS.It is worthy of further study.
