Electrocardiogram Parameters In Patients With Primary Aldosteronism

Abstract/Summary:
Objective: To compare the differences between conventional electrocardiogram and dynamic electrocardiogram in patients with primary aldosteronism(PA)and non-primary aldosteronism(non-PA),and to evaluate the rate of cardiac conduction and arrhythmia in patients with PA and relevant factors.Methods: The hospitalized patients with hypertension from 2009 to 2010 were screened and diagnosed as PA(n=161)or excluded PA(non-PA,n=435).All patients completed 12-lead ECG examination.124 patients were performed 24-hour dynamic ECG,in PA group(n=31)and non-PA group(n=93).The conventional electrocardiogram parameters,the prolongation of the PR interval and the prolongation of the QTc interval and the dynamic electrocardiogram were compared between the two groups.Correlation analysis was performed to identify factors associated with conventional ECG parameters,and regression analysis was performed to determine which parameters significantly increased the OR value of changes in ECG parameters.Results: The results of 12-lead ECG showed that the P-wave length,PR interval,QT interval and QTc interval in the PA group were longer than those in non-PA group.The detection rate of PR interval prolongation and QTc interval prolongation in PA group was higher.Changes in ECG parameters were associated with serum potassium,PRA,LVMI,and others.Having a PA increases the OR of the PR interval.The results of dynamic electrocardiogram showed that the detection rate of supraventricular arrhythmia in PA group was high.Suffering from PA increases the OR value of supraventricular arrhythmia.Conclusion: ECG changes were more pronounced in patients with PA,and the detection rates of abnormal ECG parameters and supraventricular arrhythmia are higher in PA group.And these changes were correlated with PA unique features.This may be an early manifestation of severe cardiovascular disease in patients with PA.
