Objective: To explore the correlation between cystatin C and early neurological deterioration in elderly patients with acute ischemic stroke.Methods: 140 cases of acute ischemic stroke(AIS)treated in our hospital from December 2017 to May 2018 were selected,and the general data and clinical data were analyzed.According to the level of serum cystatin C,patients were divided into 2 groups,45 in the low group and 95 in the high group.Patients were divided into 2 groups according to the National Institutes of Health Stroke Scale(NIHSS)whether had early neurological deterioration(END).Among them,53 patients in the END group and 87 cases NEND group(did not occur.END)comparing the general data,clinical data and their relationship with END.Results:A total of 140 elderly patients with acute ischemic stroke were included,including 45 in the low group and 95 in the high group;53 in the END group and 87 in the NEND group.High-group hypertension,coronary heart disease/atrial fibrillation,large atherosclerosis(LAA),history of atherosclerotic plaque and incidence of END,C-reactive protein(CRP),infection rate higher than low group Small-vessel occlusion(SVO)and uric acid were lower than those in low group,and the difference was statistically significant(P<0.05).In the END group,the history of atrial fibrillation,Cystatin C,white blood cell count,microalbuminuria,homocysteine,fibrinogen,NIHSS score at admission,incidence of atherosclerotic plaque,C-reactive protein(CRP)and cardiogenic embolism(CE)were higher than those in the NEND group.The small vascular occlusion(SVO)and triglyceride(TG)were lower than those in the NEND group.the difference was statistically significant(P < 0.05).Logistic regression analysis suggested that cystatin C,NIHSS scores at admission,incidence of infection,and elevated homocysteine levels wererisk factors for END after acute ischemic stroke in the elderly patients with normal renal function.Among them,cystatin C and NIHSS Scores at admission,incidence of infection and homocysteine were positively correlated with END.Conclusion: In elderly patients with normal renal function,Cystatin C level is associated with early neurological deterioration after acute stroke.Cystatin C level is a valuable predictor of early neurological deterioration after acute stroke.
