Objective In this study,we observed the effect of Taohong Siwu Decoction on the degree of limb swelling,D-dimer and fibrinogen in patients with intertrochanteric fracture after operation.To explore the significance of Taohong Siwu Decoction in reducing swelling and preventing deep venous thrombosis of lower limbs after intertrochanteric fracture of femur.To provide clinical basis for TCM prevention and treatment of deep venous thrombosis after lower limb fracture surgery,and optimize the prevention and treatment of thrombosis after lower limb fracture surgery.Methods The subjects were 40 patients who met the diagnostic criteria for intertrochanteric fracture of femur hospitalized in****Hospital from October,2018 to August,2019.They were randomly divided into two groups,the experimental group(Taohong Siwutang+Rivarshaban)and the control group(Rivarshaban),with 20 cases in each group.Skin traction was performed after definite diagnosis.Preoperative comprehensive treatment was adopted to prevent DVT of lower limbs in both groups.PFNA internal fixation was performed by the same group of doctors.The experimental group was given Taohong Siwu Decoction orally 12 hours after operation,twice a day in the morning and evening,and Rivaroxaban tablets 10 mg once a day 10 hours after operation,while the control group was given Rivaroxaban tablets 10 hours after operation,10 mg once a day.Anti-infective therapy was given in both groups 24-48 hours after operation,and appropriate rehabilitation training was given after operation.The circumferences of lower limbs were measured 2 days before operation and 1,3,7 and 10 days after operation.The distal patella of the affected limbs was 15 cm above the upper edge of the patella and 10 cm below the lower edge of the patella.Venous blood was drawn 2 mL before operation and 1,3,7 and 10 days after operation.D-dimer and fibrinogen were detected.Perimeter,D-dimer and fibrinogen content were recorded and analyzed.Data were analyzed by SPSS 19.0 software.Results There was no significant difference in age,sex,basic diseases and other general data between the two groups,and there was no significant difference(P>0.05).The change trend of lower extremity swelling degree in the two groups was basically the same.The thigh and leg circumference reached the highest point in 1 and 3 days after operation.Compared with the control group,the speed of swelling reduction in the experimental group was more obvious than that in the control group.On the 7th day,the thigh circumference in the experimental group was significantly smaller than that in the control group(P<0.05),and the change range in the control group was larger than that in the experiment group(P<0.05).The group is smaller.Comparing the circumference of leg,the change trend of lower limb swelling speed in the two groups was basically the same.The swelling of lower limb in the experimental group decreased slightly faster than that in the control group.The D-dimer content of the two groups reached the highest point on the first day after operation.The D-dimer content of the two groups decreased significantly on the third day after operation compared with the first day after operation(P<0.05).The D-dimer content of the experimental group was significantly lower than that of the control group(P<0.05),and the decrease was faster than that of the control group.During the period from 3 to 10 days after operation,the decrease of D-dimer content in the experimental group was relatively slow,while that in the control group remained relatively stable.The content of fibrinogen reached its peak one day after operation,and the decline rate of fibrinogen in the two groups was basically the same,which was significantly lower than that before operation.There was no significant difference between the two groups(P>0.05).In the experimental group,fibrinogen content was significantly lower than that of the previous sampling at 3 and 10 days after operation(P<0.05),while in the control group,only one sample test at 7 days after operation was significantly lower than that of the previous sampling(P<0.05).Conclusion The application of Taohong Siwu Decoction is conducive to the reduction of swelling in intertrochanteric fracture of femur,and effectively reduces the risk of deep vein embolism of lower limbs.
