Objective:To explore the characteristics of diagnosis and treatment of Haglund syndrome and the choice of surgical methods,summarize experience and experience,and improve the understanding of the disease.Method:From January 2012 to June 2018,the clinical data of patients with Haglund syndrome admitted to Hand and Foot Surgery Department of Bethune First Hospital of Jilin University and Branch Hospital of Bethune First Hospital of Jilin University were analyzed.According to the inclusion criteria and exclusion criteria of this study,36 patients were selected.The American Foot and Ankle Association(AOFAS)ankle and hind foot scoring tables were compared before and after surgery.Visual analogue scoring(VAS)was used for scoring and statistical analysis.Result:A total of 36 patients were enrolled in this study.All patients were followed up.The wounds healed in one stage,including 12 males(33%)and 24 females(67%).The ratio of male to female was 1:2.Thirty-six patients,aged 13-71 years,with an average age of 38.94(+13.84 years).There were 3 patients under 20 years old,20 patients between 20 and 40 years old(including 40 years old),9 patients between 40 and 60 years old(including 60 years old),and 4 patients over 60 years old.The VAS score of 36 patients was 7.03(+1.81)before operation and 2.03(+0.94)after operation.There was significant difference before and after operation(P=0.000<0.05).The AOFAS score was 46.7(+3.41)before operation and 91.56(+5.25)after operation.There was significant difference before and after operation(P=0.000<0.05).The AOFAS score after operation was significantly higher than that before operation,and the VAS score after operation was significantly lower than that before operation.Twelve patients underwent Achilles tendon insertion debridement combined with thread anchor for Achilles tendon insertion reconstruction.Twenty-four patients underwent simple posterior superior calcaneal nodule resection by traditional method.AOFAS ankle and hind foot scores and VAS scores were compared before and after operation.The long-term effect of resection of posterior superior calcaneal nodule and reconstruction of Achilles tendon insertion was better than that of simple posterior superior calcaneal nodule resection.Conclusion:1.Haglund syndrome includes Achilles tendinitis,Haglund malformation and Achilles tendon bursitis.The concepts of these three diseases can not be confused.2.The diagnosis of Haglund' s syndrome includes clinical manifestations and X-ray examination.When the diagnosis of Haglund's syndrome is not clear,the diagnosis should be confirmed by MRI.3.Operative methods include open surgery and arthroscopic surgery.At present,open surgery is the main treatment.Anchor fixation is needed when calcification of the Achilles tendon insertion is excessive.The long-term effect of resection of posterior superior calcaneal tubercle and reconstruction of the Achilles tendon insertion is better than that of simple resection of posterior superior calcaneal tubercle.
