Objective:This study aims to investigate the value of plasma antithrombin-III(ATIII)activity,procalcitonin(PCT)and hypersensitive c-reactive protein(hs-crp)levels in the treatment of neonatal severe sepsis,as well as its correlation with the severity of the disease and to evaluate the prognosis.Methods:A retrospective analysis was conducted to collect 50 cases of infants admitted to the Neonatal intensive care unit(NICU)of our hospital from January 2016 to December 2018 who met the diagnostic criteria for Neonatal severe sepsis,were hospitalized for more than 7 days,and had complete medical records.46 cases of infants with general infection were selected as the study subjects.Peripheral venous blood was collected on day 1,day 3 and day 7 after the diagnosis of severe sepsis in all children.Among them,serum antithrombin-III was detected by hair substrate method,PCT and hs-crp were detected by immunofluorescence dry quantitative method,and grouping comparison and correlation analysis were performed.Results:1.Comparison of gestational age,gender,weight and age between severe sepsis group and general infection group:severe Gestational age in the sepsis group and the general infection group were intermediate premature(early premature,late premature)and intermediate premature(intermediate premature,late premature),respectively.Comparison between the two groups,t was 0.483,P=0.630,and the difference was not statistically significant(P>0.05).Gender(male%,female%)were respectively(64%,36%),(52.2%,47.8%),and t was compared between the two groups was 1.37,P=0.24,and the difference was not statistically significant(P>0.05).Their weights were(2.16 0.79)kg and(2.08 0.83)kg,respectively.T was compared between the two groups was 0.47,P=0.12,and the difference was not statistically significant(P>0.05).The mean age at the onset of severe sepsis was(5.3 2.21)days and(5.11 2-43)days,respectively.T was compared between the two groups was-1.649,P=0.102,and the difference was not statistically significant(P>0.05).2.Comparison of serum ATIII,PCT and Hs-crp between severe sepsis group and general infection group:severe Serum ATIII activity in the sepsis group and the general infection group was 24(21,25)%and 28(24.75,35.25)%,respectively.PCT values were 15(4.18,46)ng/ml and 2.5(1.85,21.5)ng/ml,respectively.Hs-crp was 9.1(2.23,23.00)mg/L and 2.3(1.3,11.25)mg/L,respectively.Z=-2.565 and P=0.010 were compared between the two groups,and the difference was statistically significant(P<0.05).3.Among the 50 included neonatal children with severe sepsis,34 survived and 16 died.Comparison of gestational age,gender,weight,and age of day between the survival group and the death group after treatment for severe sepsis:the gestational age of the survival group and the death group were early preterm(early preterm,late preterm),intermediate preterm(early preterm,late preterm),respectively.Gender(male%,female%)were(70.6%,29.4%),(50%,50%),weight(2.22 0.82)kg,(2.04 0.74)kg,respectively.The mean age at diagnosis was(5.00 2.46)days and(5.752.46)days,respectively.Comparison of gestational age,gender,weight and age between the two groups,t were 1.23,2.01,1.02,-0.74,p>0.05,the difference was not statistically significant.4.Serum levels of the survival group and the death group were detected on day 1,3 and 7 of the diagnosis of severe sepsis,respectively comparison of AT?,PCT and Hs-crp:on the first day after the occurrence of severe sepsis,AT? activity decreased,while PCT and Hs-crp levels increased significantly.There was no statistical difference between the two groups(P>0.05).ATIII activity was lower in the death group AT day 3 and day 7,and Hs-crp level was higher and higher.Although PCT level decreased,it was still higher than that in the survival group,which was statistically significant(P<0.05).5.Correlation analysis of severe sepsis death:the correlation coefficients of?,PCT,and Hs-crp were r=-0.75,0.49,and 0.71,respectively,which were closely related to the decrease of AT? and the increase of Hs-crp(p<0.05).Conclusions:1.Serum antithrombin-?(AT?),like PCT and Hs-crp,can be used for severe sepsis early warning indicators.2.There is a good correlation between AT? in severe sepsis and PCT and Hs-crp,and the decrease of AT? in severe sepsis and the significant increase of PCT and Hs-crp can reflect the severity of sepsis.
