Objective To observe the changes of proinflammatory cells in patients with benign prostatic hyperplasia complicated with chronic prostatitis.In this study,we determined the distribution and infiltration of B cells,macrophages,CD4+ T cells and CD8+ T cells in the whole section of BPH patients in Tibetan Plateau,and compared them with the clinical parameters of the plain group.To explore the effect of inflammatory cells on the clinical symptoms of benign prostatic hyperplasia in Tibet Plateau.Through the expression of B cells,macrophages,CD4+ T cells and CD8+ T cells in the tissues of patients with benign prostatic hyperplasia in Tibet Plateau,the basis for the pathogenesis of benign prostatic hyperplasia caused by chronic inflammatory cells in the plateau is provided,which is a benign prostate in the Tibetan Plateau.Patients with hyperplasia provide better treatment options.Methods A total of 50 Tibetan patients in the plateau with BPH diagnosis and TURP surgery,aged 50-90 years,were selected from the Department of Urology,People's Hospital of Tibet Autonomous Region from 2015 to 2018.A total of 50 Tibetan patients in the plains with BPH diagnosis and TURP surgery,aged 50-90 years,were enrolled in the Department of Urology,Peking University First Hospital from 2015 to 2018.50 cases of TURP surgery were included in the criteria:detailed medical records,IPSS scores,QOL scores,and clinical examination reports,including urine flow rate tests,etc.,including exclusion criteria:suspected prostate cancer(PC)or postoperative Confirmed as prostate cancer(PC).The collection of 50formalin-fixed paraffin-embedded prostatic hyperplasia specimens from TURP was performed to observe the distribution of B cells,macrophages,CD4+T cells and CD8+T cells in the specimens by immunohistochemistry.And the difference in density,according to the distribution of inflammatory cells and the relative position of the glandular epithelium of the prostate tissue are divided into three types:gland-based,glandular and gland-interstitial.All data were analyzed by SPSS 23.0 software,and the quantified results were expressed as mean±standard deviation(X±S),The t-test of two-mean comparison and the analysis of variance of the three-mean comparison.P<0.05 indicates statistical significance,and P<0.01 indicates significant difference.Results Among the 50 patients with chronic prostatic hyperplasia complicated with chronic prostatitis in the high altitude group,the proportions of gland-based classification of mild inflammation,moderate inflammation and severe inflammation were 48.0%,36.0% and 16.0%,respectively.The proportions of mild inflammation,moderate inflammation and severe inflammation were 50.0%,20.0%and 30.0%,respectively;gland-based and glandular and interstitial types were compared,the difference was statistically significant(P<0.05),Among the glandular and interstitial types,the difference was statistically significant(P<0.05).Among the 50 patients with chronic benign prostatic hyperplasia and chronic prostatitis in the plain group,the proportions of gland-based classification of mild inflammation,moderate inflammation and severe inflammation were 54.0%,38.0% and 8.0%,respectively;The proportions of degree inflammation,moderate inflammation and severe inflammation were 46.0%,28.0% and 26.0%,respectively;gland-based and glandular and interstitial types were compared,the difference was statistically significant(P<0.05),Among the glandular and interstitial types,the difference was statistically significant(P<0.05).The levels of B cells,macrophages,CD4+ T cells and CD8+ T cells in the 3 degree group of benign prostatic hyperplasia in the high altitude group and the plain group were significantly higher than those in the 2 degree group.The levels of B cells,macrophages,CD4+ T cells and CD8+ T cells in the 2nd group were significantly higher than those in the 1 degree group,and the differences were statistically significant(P<0.01).At present,50 cases of benign prostatic hyperplasia patients collected in the Tibetan Plateau and Plains group showed complete compliance with the diagnostic criteria of chronic prostatitis.There were 17 cases(34%)in the 1 degree group and 17 cases(34%)in the 2 degree group.There were 16 cases(32%)in the 3 degree group.Among them,the levels of B cells,macrophages,CD4+ T cells and CD8+ T cells in the specimens were higher than those in the plain group,and the difference was statistically significant(P<0.05).Conclusion The proinflammatory cytokines in high altitude group and plain group are highly expressed in patients with chronic benign prostatic hyperplasia.The levels of B cells,macrophages,CD4+ T cells and CD8+ T cells affect the occurrence and development of benign prostatic hyperplasia.Infiltration of inflammatory cells is common in tissue specimens of patients with benign prostatic hyperplasia in the Tibetan Plateau.The levels of B cells,macrophages,CD4+ T cells and CD8+ T cells in the plateau specimens are higher than those in the plains.The geographical environment affects the occurrence and development of BPH,and hypoxia may be an important factor.
