Objective: To evaluate the associations of both physical activity(PA)and PA type with incident cardiovascular disease(CVD)and to provide evidence for CVD prevention among middle-aged and elderly Chinese. Methods: All the participants came from the first follow-up survey of DFTJ cohort(since April 2013 to December 2013),including 24175 participants who finished the first follow-up survey in 2013 and 14120 participants who were newly enrolled in the first follow-up.We excluded those with relevant diseases, missing PA information and PA outliers.During 3.59 ± 0.45 years follow-up,we included 26225 participants(3185 incident CVD,2624 incident CHD and 561 incident stroke cases). In the association of PA amount with incident CVD,26225 subjects were grouped by tertiles of total PA amount into low, moderate and high PA groups. In the association of PA type with incident CVD,after being excluded 312 subjects without major PA type,25913 participants were included and were grouped by tertiles of major PA amount into low, moderate and high PA level.Cox proportional hazard models were used to estimate the hazard ratios(HR)and 95% confidence intervals(CI).Results: In the association of PA amount and incident CVD,compared with those without PA,the adjusted HRs(95% CIs)of those with moderate baseline PA level were 0.83(0.74,0.94),0.89(0.78,1.01)and 0.64(0.48,0.84)for CVD,CHD and stroke;0.77(0.61,0.98)and 0.67(0.49,0.91)for acute coronary syndrome(ACS)and ischemic stroke(IS)respectively.Compared with those without PA,the adjusted HRs(95% CIs)of those with high baseline PA level was 0.82(0.73,0.93),0.87(0.75,0.99)and 0.69(0.52,0.93)for CVD,CHD and stroke;0.74(0.58,0.96)and 0.67(0.48,0.93)for ACS and IS respectively. In the association of PA type and incident CVD,compared with those without PA,the HRs(95%CIs)of cycling and jogging were 0.54(0.33,0.90)and 0.48(0.25,0.92)in the group of low major PA amount, respectively.Compared with those without PA,the HRs(95% CIs)of square dancing and cycling were 0.58(0.40,0.84)and 0.47(0.25,0.89)in the group of moderate major PA amount, respectively.Compared with those without PA,the HRs(95% CIs)of walking and square dancing were 0.73(0.56,0.94)and 0.87(0.77,0.99)in the group of high major PA amount, respectively. Conclusions: Moderate to high baseline PA level was inversely associated with the risk of CVD.Jogging, cycling, square dancing and walking were associated with reduced CVD risk among middle-aged and older Chinese. |
