Objective:To assess the clinical value of 18f-fluorodeoxyglucose PET/CT(18F-FDG PET/CT)in the diagnosis of FIGO stages ?A2-?A2 cervical cancer.Methods:The clinical data of 136 patients with ?A2-?A2 stage cervical cancer admitted to the fourth gynecological ward of our hospital from January 2015 to May 2018 were retrospectively analyzed.The sensitivity,accuracy,specificity,positive predictive value and negative predictive value of PET/CT in the diagnosis of primary cervical cancer and pelvic lymphatic metastasis were calculated respectively.The differences of primary lesion maximum standard uptake?SUVmax?between different pathological types,differentiation degree,FIGO stage and lesion size were compared.SUVmax in metastatic lymph nodes was compared with that in non-metastatic lymph nodes.By plotting ROC curve and calculating the area under the curve?AUC?,the optimal SUVmax cut-off value for the diagnosis of lymph node metastasis was further determined.Results:The sensitivity of PET/CT in the diagnosis of primary cervical cancer was100%?136/136?.The sensitivity,accuracy,specificity,positive predictive value and negative predictive value for the diagnosis of pelvic lymph node metastasis were70.0%?21/30?,83.3%?114/136?,87.7%?93/106?,61.8%?21/34?and 91.2%?93/102?,respectively.There were statistical differences in the primary cervical SUVmax between different pathological types,differentiation degree and primary lesion size,while there were no statistical differences among different FIGO stages.SUVmax of metastatic and non-metastatic lymph nodes was 6.0±4.9 and 2.8±0.3,respectively.The difference was statistically significant?P<0.05?.The optimal diagnostic threshold of SUVmax for lymph node metastasis was 3.45.Conclusion:18F-FDG PET/CT has important clinical significance in the diagnosis of primary lesions of cervical cancer,pelvic lymph node metastasis.
