Objective:To compare the diagnostic performances of ultrasonography and digital breast tomosynthesis(DBT)in women with dense breasts.Methods:A total of 148 women with dense breast underwent digital mammography with tomosynthesis and ultrasonography for diagnostic purposes.According to the composition of the breast,age,size of the lesion and the presence or absence of calcification to group,and the pathological results are the gold standard.The diagnostic performance of two examination were evaluated by ROC curve,sensitivity and specificity.Results:Based on the area under curve(0.956 vs 0.815,Z=3.866,P<0.001),ultrasonography showed better diagnostic performance in women with dense breast than DBT.There were significant difference in AUC between ultrasonography and DBT among heterogeneously dense,lesions which are no more than 1 cm,patients aged?40years or patients aged<40~55 years(0.967 vs 0.821,P<0.001;0.935 vs 0.680,P=0.022;0.973 vs 0.817,P=0.030;0.946 vs 0.801,P=0.013).According to clinical diagnostic criteria,the sensitivity of ultrasound and DBT in the diagnosis of benign and malignant breast lesions was 94.32%(83/88)and 93.18%(82/88),the difference was not statistically significant(?~2=0.096,P=1.000);Specificity was 83.33%(50/60)and 61.67%(37/60),the difference was statistically significant(?~2=7.06,P=0.007).Conclusion:Ultrasonography showed better diagnostic perfomance than DBT in women with dense breast.Small lesions,heterogeneously dense breasts,and patients aged?55years are more suitable for ultrasound as the preferred examination.
