Objective : To investigate the general clinical features of BCR-ABL negative myeloproliferative neoplasms(MPNs)and related factors of thromboembolism,and to guide clinical work.Methods : Retrospective analysis of clinical data of 165 patients with BCR-ABL negative myeloproliferative tumors admitted to Shengjing Hospital of China Medical University from October 2011 to October 2018.Age,gender,JAK2 mutation,previous history of thromboembolism,blood cell count,coagulation,etc.were used as observation indicators for univariate analysis,and logistic regression was used for correlation analysis.Results:1)In three BCR-ABL negative MPNs,including essential thrombocytemia(ET),polycythemia vera(PV),and primary myelofibrosis(PMF),the incidence of PV is 50.91%,and it is the highest,and the ratio of male to female in PV and ET patients is(1.15:1)and(0.40:1).2)Of the 165 BCR-ABL negative MPNs patients,31(18.79%)had a PT prolongation,41(24.85%)had a APTT prolongation,52(31.52%)had a PTA reduction,and 42(25.45%)had a rise in INR,17(10.30%)patients had a FIB reduction,and 21(12.73%)patients had a higher DD.3)The incidence of thromboembolic events in BCR-ABL negative MPNs was32.73%.The incidences of cerebral embolism,cardiovascular embolism and extremity venous thrombosis were 77.78%?11.11% and 11.11%,respectively.4)Univariate analysis showed that there were significant differences between the thromboembolic group and the non-thromboembolism group in the patients with hypertension,previous history of thromboembolism(P<0.05).5)Multivariate logistic regression analysis showed that the risk of thromboembolism in BCR-ABL negative MPNs with a history of hypertension was 2.790 times higher than that in patients without a history of hypertension.The risk of thromboembolismin BCR-ABL negative MPNs with a history of thromboembolism was about 2.235 times higher than that in patients without a history of thromboembolism,suggesting that hypertension and previous history of thromboembolism are independent risk factors for thromboembolic events in patients with BCR-ABL negative MPNs(?>0,OR>1,P<0.05).Conclusion:1)The incidence of PV is the highest among PV,ET and PMF,and it is the most common type.PV patients have more men than women and ET patients have more women than men.2)Patients with BCR-ABL negative MPNs complicated with thromboembolism are more common.Arterial thrombosis is more common than venous thrombosis,the most common is cerebrovascular events.3)The history of hypertension and previous history of thromboembolism were associated with BCR-ABL negative MPNs complicated with thromboembolic events and were independent risk factors.Therefore,patients with previous history of thromboembolism and history of hypertension have a higher probability of thromboembolic events in patients with BCR-ABL negative MPNs,and active prevention is needed.In addition,patients with BCR-ABL-negative MPNs may have abnormal coagulation function,and we need to be careful about the occurrence of bleeding events in clinical work.
