Objective: To explore the long-term effects of on the pelvic floor function of the same age group after.total hysterectomyMethods: A total of 60 patients who underwent total hysterectomy for benign gynecological diseases at Shengjing Hospital affiliated of China Medical University from January 2012 to August 2015 were enrolled?Including 28 patients without premenopausal,The average postoperative follow-up time was 4.27± In 0.95 years,and 32 patients with premenopausal menopause had an average postoperative follow-up of 4.71±1.22 years.The patients with gynecological benign diseases,who was admitted to Shengjing Hospital of China Medical University.There were 61 preoperative patients,including 31 cases without menopause and 30 cases with menopause.There were 53 healthy physical examination patients in the gynaecological outpatient department of Shengjing Hospital of China Medical University,including 26 cases of non-menopausal and 27 cases of menopause.They were respectively tested in Chinese version of the pelvic floor function impact questionnaire 20 and the current sexual life satisfaction survey.The patient's current pelvic floor muscle fiber strength,muscle fatigue,vaginal dynamic pressure and anterior and posterior vaginal wall bulging rate were measured,and the current pelvic floor function was evaluated from a subjective and objective erspective.Results: The age,parity and preoperative uterus size of the three groups of non-menopausal and menopausal patients were not significantly different,P>0.05 was not statistically significant and comparable.1.Comparison of subjective indicators:(1)Non-menopausal patients: UDI-6 scores in the postoperative group were significantly higher than those in the control group and the preoperative group,and the difference was statistically significant(P<0.05).There was no significant difference in the scores of the three groups of POPDI-6 and CRADI-8.,no statistical significance(P>0.05).There was no significant difference in the current sexual life satisfaction rate between the control group and the preoperative group(P>0.05),but they were better than the postoperative group,and the difference was statistically significant(P<0.05).(2)Postmenopausal patient group: The scores of CRADI-8 and CUDI-6 in the postoperative group were significantly higher than those in the control group and the preoperative group,and the difference was statistically significant(P<0.05).There was no significant difference in the scores of the three groups of POPDI-6.(P>0.05).The satisfaction rate of the current control group was 81.5%(22/27).There was no significant difference in the current satisfaction rate between the three groups(P>0.05).2.Comparison of objective indicatorsThere were significant differences in the type I and II muscle between the control group and the control group,the preoperative group and the postoperative group(P<0.05),and the I and II muscle strengths in the control group and the preoperative group were excellent.The postoperative group(P<0.05);there was no significant difference between the control group and the preoperative group I and II muscle strength(P<0.05).There was no significant difference in muscle fatigue between the three groups of type I muscles(P>0.05).The muscle fatigue of class II muscles in the control group and the preoperative group was higher than that in the postoperative group,the difference was statistically significant(P<0.05)..There was no significant difference in the anterior and posterior wall bulging rate between the three groups(P>0.05).The vaginal dynamic pressure of the non-menopausal patients in the control group and the preoperative group was higher than that in the postoperative group(P<0.05).There was no significant difference in the vaginal dynamic pressure between the preoperative group and the postoperative group.The dynamic pressure was higher than that in the postoperative group,and the difference was statistically significant(P>0.05).Conclusion: Non-menopausal patient group: Total hysterectomy has a negative impact on long-term urination function and sexual life satisfaction in this age group,and has little effect on pelvic organ prolapse and intestinal function;surgery on pelvic floor I muscle,Type II muscle strength II muscle fatigue decreased,and had no significant effect on type I muscle fatigue and vaginal dynamic pressure.Postmenopausal patient group: Laparoscopic hysterectomy has a negative effect on long-term urination function and intestinal function in this age group,and has little effect on pelvic organ prolapse and sexual life satisfaction;In the former patients,the muscle strength of type I and type II muscles of pelvic floor decreased,and the effects on muscle fatigue and vaginal dynamic pressure of type I and type II muscles were not obvious.Therefore,preoperative patients should be treated individually according to the patient's age,condition,and quality of life in the treatment of benign uterine diseases,and the uterus should be removed reasonably.
