Objective: To explore the correlation between ankle brachial index and serum homocysteine level in patients with lower extremity atherosclerotic occlusion and the risk factors of hyperhomocysteinemia,and to provide new ideas for the treatment and research of lower extremity atherosclerotic occlusion.Methods: From January 2017 to December 2018,87 patients with lower extremity atherosclerotic obliteration were collected from vascular surgery department of the first affiliated Hospital of China Medical University.All patients underwent ankle brachial index measurement,vascular ultrasound and lower extremity CTA examination to confirm the diagnosis of lower extremity atherosclerosis obliteration.The patients' symptoms and Fantaine grading were recorded,and the history of hypertension,diabetes,coronary heart disease,smoking and drinking were recorded,and fasting blood samples were taken on the second day of admission.Serum homocysteine(hcy),total cholesterol(tc),triglyceride(tg),low density lipoprotein(ldl),high density lipoprotein(hdl)C reactive protein(Crp)et al.Results: 1.Homocysteine was negatively correlated with ankle-brachial index(B=-2.72,P=0.011).2.In patients with lower extremity atherosclerotic occlusion,there was a significant difference in ankle-brachial index between hyperhomocysteinemia group and normal homocysteine group(P=0.021).The ankle-brachial index,diabetes mellitus,coronary heart disease and smoking were analyzed by multivariate logistic regression.Except for protective factors,other factors were risk factors.3.Patients with lower extremity atherosclerosis occlusion were classified according to Fontaine stage.There were significant differences in ankle-brachial index(P < 0.001),C-reactive protein(P <0.001)and homocysteine(P < 0.001)between mild group and severe group.Conclusion: There was a significant negative correlation between ankle brachial index and plasma homocysteine level in patients with lower extremity atherosclerosis occlusion.The independent risk factors of hyperhomocysteinemia were ankle brachial index,coronary heart disease and smoking.The protective factor was diabetes mellitus.Symptoms were negatively correlated with ankle brachial index.
