Objective: Membranous nephropathy" is not only the most common cause of adult nephrotic syndrome,but also the common cause of(ESRD)in end-stage renal disease.About 80% of membranous nephropathy is idiopathic membranous nephropathy(IMN).IMN is an antibody-mediated autoimmune disease specific to kidneys,which cause is unknown.In recent years,the study of micro RNA has gradually become a hot spot,and it has been found that micro RNA may be involved in the occurrence and development of IMN at the translation level.In this study,we aimed to detect the expression of mi RNA150 in peripheral blood monocytes(PBMC)of IMN patients and normal control to explore the expression differences of mi RNA150 of PBMC in patients with IMN.To further analyze the changes in expression of IMN patients with hormone and cyclophosphamide shock therapy,and to observe its correlation with a number of clinical biochemical indicators.Methods: Peripheral blood mononuclear cells(PBMC)were collected from 22 patients with IMN confirmed with PLA2 R positive by renal biopsy and 12 healthy controls from2015 to 2016,total RNA in PBMC was extracted and the expression level of mi RNA150 in PBMC was detected by real-time fluorescence quantitative PCR(QPCR),Graph Pad Prism 5 was used for analyzing the expression differences of mi RNA150 in healthy control group and IMN patients,when P < 0.05 was considered statistically significant.Among them,8 pairs of follow-up patients were selected to separately measure the level of mi RNA150 in the PBMC of 0g and 3g treated with cyclophosphamide(CTX),and analyze the change of mi RNA150 with the treatment.Results: 1.Compared with the healthy control group,the expression of mi RNA150 in PBMC of patients with PLA2 R positive IMN showed an upward trend,and the data has a statistically significant difference.2.In 8 patients with PLA2 R positive IMN with complete follow-up,the level of mi RNA150 in PBMC decreased gradually with hormone and cyclophosphamide shock therapy to 3g,which can accurately reflect the initial untreated and CTX cumulative 3g remission of the disease.3.In incipient not cure PLA2 R positive IMN patients,mi RNA150 in PBMC and 24 hours Urinary protein quantitative(Urinary total proteinuria,URTP),LDL-c,PLA2 R positive correlation(P <0.05),and Serum albumin,s ALB)negative correlation(P < 0.05).Conclusion: The expression of mi RNA150 in PBMC of PLA2 R positive IMN patients was significantly increased,and as the hormone with CTX shock therapy to 3 g,mi RNA150 expression decreased,this study may support the future application of mi RNA150 as a diagnostic biomarker and therapeutic target,which may be a new monitoring indicator for response to therapeutic effects.
