Objectives: To investigate the ankle joint stability and function after the effect of middle and upper part of fibula resection in avascular femoral head necrosis patients with vascularized fibular graft treatment over two years follow up via in vivo biomechanical testing.Materials and Methods: Between January2016 to December 2016,raised 20 patients with femoral head necrosis more than two years after surgery;excluding muscle neurological diseases and walking disabilities.Before test inquiry history,height,weight,BMI,dominant side,the entire length of both lower extremities,and the length of the circumference of the thigh,foot circumference;the main skeletal muscle of the lower limbs were measured.The gait test was performed by using the motion capture system.The gait test was performed at 0.39 m / s-1.50 m / s(increments 0.222 m / s)and 6 different speeds respectively.The time and space parameters of the patients were recorded.Ankle kinematics and kinetic parameters,electromyography(EMG)were recorded;0.61 m / s,1.06 m / s,1.50 m / s,3 different speeds of gait pain VAS score were recorded.Results: In the time-space parameters of the patients with more than 2 years postoperatively,the mean step size of the operated side was smaller than non-operated side only at 0.39m/s the difference was statistically significant(P <0.05),and the residual parameters were not significantly different.Kinematic and kinetic parameters more than 2 years after surgery in patients' operated side and non-operated side were compared,the differences were not statistically significant(P> 0.05).Patients' EMG after 2 years was roughly consistent in operated side between non-operated sides.Conclusion: Ankle joint biomechanics in the treatment of femoral head necrosis with middle and upper part of fibular graft after two years basically had no change,in which the kinematics,kinetics and EMG parameters have reached normal levels.
