?Objective? To analyze the feasibility, safety efficacy of carotid artery stenting in patients with chronic internal carotid artery occlusion. And to analyze the value of HR-MRI in the treatment of chronic internal carotid artery occlusion, then to determine predictors for successful endovascular treatment in patients with chronic carotid artery occlusion.?Materials and methods? The clinical data of patients with chronic internal carotid artery occlusion underwent endovascular recanalization at Renji Hospital affiliated to Shanghai Jiaotong University school of medical, from January 2009 to July 2015 were analyzed retrospectively. The perioperative complications and clinical efficacy were analyzed. We also used HR-MRI to evaluate the characteristics of the lumen of internal carotid artery occlusion. Then we analyzed potential variables in relation to the technical success.?Results? Successful recanalization was achieved in 41 patients.Intracranial hemorrhage was occurred in one patient, without any sequelae. In the follow-up,five patient suffered frequent TIA.Follow-up imaging revealed re-occlusion developed in only one patient. And there are some potential variables in relation to the technical success.?Conclusions? Stent-assisted endovascular recanalization is feasible and safe for patients with symptomatic chronic internal carotid artery occlusion,it has a low perioperative stroke rate and decent outcome on follow-up.HR-MRI has important value in evaluating the morphological characteristics of the lumen of internal carotid artery occlusion. And it may play a role in predicting successful rate and 1-year patency for endovascular recanalizationin carotid CTO.Absence of prior neurologic event, nontapered stump, distal ICA reconstitution via contralateral injection, and distal ICA reconstitution at communicating or ophthalmic segments were identified as independent negative predictors for technical success in endovascular recanalization for CAO...
