Objective: To explore the predictive factors of prostate cancer(PCa)and its poor prognosis,to reduce the unnecessary biopsies and evaluate the prognosis of PCa,to pay way to precise diagnosis and personalized prognostic evaluation.Methods: From Jul 2014 to Aug 2016 data on 1144 patients who had undergone prostate biopsies were prospectively collected and analyzed.From Jan 2010 to Dec 2014 data on 290 PCa patients who had undergone endocrine therapy as first-line therapy were retrospectively analyzed.Univariate and multivariate logistic regression analyses were performed to explore the independent predictors for PCa and high-progression PCa(HPPCa).MiRNA sequence and microarray were performed to explore the serum exosome derived miRNAs for the diagnosis of PCa and metastatic PCa.Univariate and multivariate cox regression analyses were performed to investigate the independent predictors for the poor prognosis of PCa.Results: Multivariate logistic regression analyses identified prostate transitional zone volume(TZV)was an independent predictor for PCa and HPPCa,prostate volume(PV)and prostate volume index(PVI)were not.MiRNA sequence and microarray indicated the difference of multiple serum exosome derived miRNAs levels was significant between control,PCa and metastatic PCa.Pretreatment elevated plasma fibrinogen level,elevated platelet to lymphocyte ratio(PLR)and decreased prognostic nutritional index(PNI)were idependent predictors for the poor prognosis of PCa.Conclusion: TZV is a better predictive biomarker than PV or PVI for PCa and HPPCa.Serum exosome derived miRNAs levels could be applied for the diagnosis of PCa and metastatic PCa.Pretreatment plasma fibrinogen level,PLR and PNI were idependent prognostic indicators for PCa.
