Clinical Study Of Enhanced Recovery After Surgery In Laparoscopic And Open Radical Gastrectomy

Objective: To study the efficacy and safety of enhanced recovery after surgery(ERAS)combined with laparoscopic radical gastrectomy for gastric cancer.Methods: A total of 51 patients with gastric cancer performed D2 gastrectomy in oncology department,the first hospital of Lanzhou university from may 2018 to october 2018 were divided into group A(open gastrectomy with ERAS,n=27)and group B(laparoscopic gastrectomy with ERAS,n=24).Then we recorded their gender,age,BMI,ASA classification,resection and reconstruction methods.Results: The gender,age,BMI,American Society of Anesthesiologists(ASA),preoperative complications,surgical resection,reconstruction and tumor staging did not show significant differences between the two groups(all P>0.05).Compared with open gastrectomy with ERAS group,laparoscopic gastrectomy with ERAS group had longer operation time[(3.52±0.48)h vs.(2.56±0.52)h,P=0.000]?less intraoperative bleeding volume[(119.58±39.83)mL vs.(202.22±132.24)mL,P=0.000]?shorter skin incision[(7.13±0.73)cM vs.(14.74±1.30)cM,P=0.000] shorter time of first oral feeding[(2.84±0.19)d vs.(3.22±0.17)d,P=0.000]?shorter time of bowel function[(2.71±0.18)d vs.(3.02±0.17)d,P=0.000],shorter days of abdominal drainage[(2.35±0.45)d vs.(2.93±1.06)d,P=0.014],and and higher hospitalization costs[(54080.92±5779.65)? vs.(44552.70±5904.78)?,P=0.000].There was no significant difference in the number of lymph node dissection ? days of hospitalization?complications ?WBC and D-dimer on the first day before operation,the first day after operation and the third day after operation between the two groups(P > 0.05).Conclusions: ERAS combined with laparoscopic radical gastrectomy for gastric cancer is safe and feasible.It can promote the recovery of gastrointestinal functionafter operation,but it can not reduce the hospitalization days and hospitalization costs of patients with gastric cancer.
