Comparative Study Of The Clinical Effects Of David And Bentall Surgery On Aortic Root Dilatation

Objective To evaluate the effect of David surgery(remaining aortic root replacement with aortic valve)and Bentall surgery(root replacement with combined valved blood vessels)on aortic root dilatation surgery,and to discuss the differences between the two perioperative periods and to do Preliminary discussion.Methods Patients with aortic root dilatation disease who were admitted to our hospital from June 2016 to June 2018 were enrolled.The patient’s surgical indications were strictly controlled by the patient’s symptoms,signs,and cardiac ultrasound and CT examinations.A total of 67 patients with aortic root dilatation disease requiring surgery were collected.The different roots were divided into Bentall operation group(52 cases)and David operation group(15 cases).The heart function of the two groups,whether there was essential hypertension,the size of preoperative left ventricular cavity,Whether or not the preoperative basic data such as the dissection is matched to the propensity score,and 15 pairs of patients are paired.The clinical data were statistically analyzed and the preliminary evaluation was made on the index of intraoperative transfer time,blood transfusion,perioperative death,and cardiac function recovery.Result The aortic occlusion time of David was longer than that of the Bentall group(341.1±48.2min vs 255.0±72.7min<0.01).The total red blood cell transfusion volume in the David group was higher than that in the Bentall surgery group(9.0(8.25~14.0)U VS 6.5(3.5~11.5)UP=0.03),there was no statistically significant difference in aortic arch surgery,MVP,CABG(P>0.05).There were no statistics on total blood transfusion,secondary thoracotomy,IABP,CRRT,etc.Learning differences(P>0.05).The overall survival rate and total mortality were similar in the two groups(P>0.05).Conclusion In the recent results,the performance of the two groups is similar.The wide range of Bentall surgery is still accepted by most surgeons.However,there is no anticoagulant adverse reaction after David surgery,which is more suitable forthe quality of life and young fertility needs.Female patients,but should be performed at an experienced heart center.
