OBJECTIVE: To compare the differences in preoperative?intraoperative and postoperative data between patients undergoing totally thoracoscopic and median thoracotomy with mitral valve replacement(MVR)and tricuspid valvuloplasty(TVP).To enhance the understanding and clinical cognition of the second and tricuspid valve combined with valvular disease.Methods: The clinical data of patients with secondary and tricuspid valve valvular disease diagnosed by admission examination,cardiac ultrasound,chest X-ray and electrocardiogram were collected from February 2013 to June 2018.All patients were strict.After screening,they were divided into two groups.One group underwent complete thoracoscopic MVR+TVP(thoracic group),and the other group underwent median open chest MVR+TVP(median thoracotomy group).The preoperative general conditions,perioperative data,postoperative discharge follow-up data of the two groups were compared and analyzed.The differences between the two groups in the above data were summarized and statistical analysis was performed.Inductive analysis of the causes of data differences between the two groups of patients provides an effective theoretical basis for the choice of surgical approach for valvular disease.Results: There was no significant difference between the thoracoscopy group and the median thoracotomy group in terms of preoperative general conditions and preoperative examination results.In the total operation time,extracorporeal circulation time,aortic occlusion time,ventilator assist time,ICU stay time,chest drainage after 24 hours,perioperative blood transfusion and postoperative hospital stay,the thoracoscopic group was more excellent than the other group(P<0.05),but there was no significant difference in the early postoperative cardiac function and complications between the two groups.All patients were followed up for an average of 32 ± 6 months.There was no significant difference in follow-up data.Conclusions: There were different degrees of differences in preoperative,intraoperative,postoperative,and follow-up data between thoracoscopic group and median thoracotomy group.However,there were no differences in the follow-up data between the two groups.Both surgical methods can be used for clinical treatment of second and tricuspid valve valvular disease.The total thoracoscopic technique is superior to the middle chest opening method in terms of curative effect,as long as the surgical indications of the former are strictly controlled,and the thoracoscopic operation skills can be mastered.Complete thoracoscopic techniques are worthy of promotion in the clinic.
