Objective:To explore the changes of the apparent diffusion coefficient?Apparent diffusion coefficient,ADC?value of normal kidneys in different altitude areas and their influencing factors.And to analyze whether altitude,renal ADC value and renal function index are related,so as to reflect the changes of renal function in imaging,and lay a foundation for the pathophysiology research of high altitude kidney.Method:A total of 83 healthy volunteers?including 42 males and 41 females,aged 22-65 years?from different altitudes in Qinghai from November 2017 to December 2018 were collected and included in the inclusion and exclusion criteria.According to different altitudes,they were divided into 1,2,and 3groups,which were?2267-2980 meters,3128-3968 meters,4006-5226 meters?.The subjects underwent routine MRI and DWI?Diffusion weighted imaging?scans at Qinghai University Hospital?b=0,800s/mm2?,laboratory renal function test.Three regions of interest were selected on the renal hilum level on the post-processing workstation.The renal cortex and medulla ADC values were measured and the mean values were obtained.The renal cortical medullary ADC values and renal function indexes of normal people at different altitudes were compared.Result:1.General results:1.2.3 renal cortex was?2.15±0.19?×10-3mm2/s,?2.03±0.07?×10-33 mm2/s,?1.89±0.12?×10-33 mm2/s.renal medulla was?1.98±0.09?×10-3mm2/s,?1.86±0.13?×10-33 mm2/s,?1.67±0.07?×10-3mm2/s.2.At the same altitude,gender had no effect on the ADC value of the kidney,and there was no difference in statistics?P>0.05?,but the ADC value of the female kidney was higher than that of the male;the ADC value of the ipsilateral renal cortex and medulla was significantly different,and the cortical ADC value Greater than the medullary ADC value,statistically significant?P<0.05?.3.The ADC values between the 3groups increased with altitude,and the ADC values between the ipsilateral cortex and the medulla gradually decreased,and there was significant difference?P<0.05?.4.Altitude was negatively correlated with normal renal cortex and medullary ADC values(r cortex=-0.794,p cortex<0.05,r medulla=-0.870,p medulla<0.05),positively correlated with uric acid and urea content(r Uric acid male=0.399,p uric acidmale<0.05,r Uric acid female=0.351,p uric acid female<0.05,r urea=0.349,p urea<0.05).renal cortical ADC value was negatively correlated with uric acid and urea content(r uric acid male=-0.401,p uric acid male<0.05,r uric acid female=-0.379,p uric acid female<0.05,r urea=-0.278,p urea<0.05).medulla was also negatively correlated with uric acid and urea content(r uric acid male=-0.255,p uric acid male<0.05,r uric acid female=-0.293,p uric acid female<0.05,r urea=-0.295,p urea<0.05).Altitude and renal cortex,medulla There was no significant correlation between ADC value and creatinine?P>0.05?.Conclusion:Kidney DWI imaging should consider the effect of altitude and internal structural factors on ADC values.Altitude,renal ADC values and the presence of laboratory renal function indicators,suggesting changes in renal function at the molecular imaging level.MRI-DWI technology can be used as a sensitive technique for judging and studying changes in renal function by clinical measurement and analysis.Because of its repetitive and non-invasive,it has a good application prospect in renal function evaluation.It lays a foundation for the study of renal pathophysiology in the plateau.
