Retrospective Analysis Of The Effect Of Gallbladder-Preserving Cholecystolithotomy For Cholecystolithiasis In Qinghai Province

Objective: To know about the performing situation of gallbladder preserving cholecystolithotomy in Qinghai province.To calculate the recurrence rate of gallstone after gallbladder preserving cholecystolithotomy and explore risk factors for recurrence of gallstones.Methods: Clinical data of patients who had received gallbladder preserving cholecystolithotomy were studied from January 2012 to January 2018 in Qinghai province.Patients were followed up to know about their postoperative conditions,and patients were divided into two groups according to whether there was recurrence or not.The cumulative recurrence rate was calculated according to the Kaplan–Meier analysis.Found risk factors of recurrence after gallbladder preserving cholecystolithotomy by statistical analysis.Results: Of these,14(9.66%)patients experienced a recurrence with a mean follow up time of 39.72±24.44 months.The mean time to recurrence was 30.07±21.21 months.The estimated 1-year,3-year and 5-year recurrence rate for all patients was 2.4%,7.8% and 12.5% respectively.Univariate analysis showed that pregnancy history(P=0.008),the homogeneity of gallstones(P=0.002),preoperative inflammation(P=0.022),postoperative oral drugs(P=0.022)and regularity of diet style(P=0.001)were significantly related to recurrence.The homogeneity of gallstones(odds ratio [OR] 0.079;95% confidence interval [CI] 0.010-0.590;P= 0.013)and regularity of diet style(OR 0.074;95% CI 0.010-0.528;P= 0.009)were independent prognostic factor for recurrence using multivariate analysis.Conclusions: Gallbladder preserving cholecystolithotomy might exist the recurrence of gallstones.The homogeneity of gallstones and regularity of diet style were independent risk factors for gallstones recurrence.
