Objective:To investigate the correlation between the expression of IL-6,TNF-? and HMGB1 in perirenal adipose tissue and the clinical indexes and prognosis of patients with renal tumor.Methods:To collect the clinical data of 50 patients with renal tumor undergoing partial nephrectomy or radical nephrectomy in Ningxia Medical University from January 2016 to March2019,and to collect samples of perirenal fat,renal tumor tissue and adjacent renal tissue.MAP was scored according to preoperative abdominal CT results and was divided into low MAP group(0-3),and high MAP group(4-5).According to preoperative imaging and intraoperative findings,all patients were divided into adherent fat group and non adherent fat group.Detection of lymphocyte infiltration in renal tumor tissue,adjacent renal tissue and perirenal adipose tissue by HE staining.Expression of IL-6,TNF-? and HMGB1 in paraneoplastic adhesions,normal adipose tissues,renal tumor tissues and adjacent renal tissues of 50 patients with renal tumors were analyzed by western blot and immunohistochemical staining.The expression of IL-6,TNF-? and HMGB1 in different MAP groups was analyzed,and the expression of IL-6,TNF-? and HMGB1 in adhesively adipose group and non-adhesively adipose group was analyzed.Comparison of the expression of IL-6,TNF-? and HMGB1 in renal tumors and adjacent renal tissues Analysis of the correlation between IL-6,TNF-? and HMGB1 and clinical indicators and prognosis.Results:IL-6,TNF-? and HMGB1 were mainly expressed in adipocytes in the non adherent fat group and in adipocytes,extracellular matrix and inflammatory cells in the adhesive fat group.Theexpression of IL-6,TNF-? and HMGB1 in different MAP groups was significant(P < 0.05).the conveys of IL-6,TNF-? and HMGB1 in the high MAP group was significant difference compared with the minuent group of MAP(P<0.05).There was no obvious diversity in the voice of IL-6,TNF-? and HMGB1 between the same MAP grades(P>0.05).The shows of IL-6,TNF-? and HMGB1 in the adherent fat group was obviously more than those in the non-adhesive fatty tissue group(P <0.05).There were significant differences in duration of surgery,amount of bleeding volume in the surgery,hospital days after surgical operation,sex difference,clinical stage and Furman grade among the two samples(P <0.05).There were no clinical significant differences such as age,operation mode,creatinine change range,Triglyceride,blood sugar,relative value of neutrophils and platelet count between the two groups(P > 0.05).The expression levels of IL-6,TNF-? and HMGB1 in renal tumors were significantly higher than those in adjacent renal tissues(P < 0.05).IL-6 was closely correlated with operation time(0.516,P = 0.000),intraoperative bleeding volume(0.429,P = 0.002),intraoperative bleeding volume(0.429,P = 0.002),postoperative hospitalization days(0.327,P =0.021),Furman grade(0.385,P = 0.011),PFS(-0.318,P = 0.024).TNF-? was closely correlated with operation time(0.385,P = 0.006,P = 0.006),intraoperative bleeding volume(0.340,P = 0.016),post-hospital days(0.280,P=0.049),Furman classification(0.453,P=0.001),relative neutrophil value(0.280,P=0.049).HMGB1 was closely related to operation time(0.376,P = 0.007),intraoperative bleeding volume(0.322,P = 0.023),postoperative hospital stay(0.285,P = 0.045),Furman grade(0.425,P = 0.002).Conclusion:IL-6,TNF-? and HMGB1 in perirenal adipose tissue may affect the clinical parameters of patients with renal tumors by participating in inflammatory reactions;IL-6 levels in perirenal tissue may be prognostic factors in patients with renal tumors.
