Objective A large number of studies have show that white Matter Lesions(WML)may lead to cognitive impairment and play a role in the etiology of dementia.However,there is still a lack of unified conclusions and interventions on the characteristics of cognitive impairment caused by WML.In this study,the relationship between visual rating according to Fazekas and the characteristics of cognitive impairment were analyzed,in order to achieve early diagnosis and prognosis assessment of patients with WML with cognitive decline.Methods During the period from January 2018 to February 2019,a total of 75 cases of WML cases and no WML cases were confirmed by cranial MRI in the outpatient department and inpatient department of Ningxia Medical University General Hospital Cardiovascular and Cerebrovascular Disease Hospital were collected,53 cases(24 males and 29 females)of WML patients were included,and set as the case group.The visual rating according to Fazekas method was used to classify WML patients in the case group,there were 24 patients in the mild group(Fazekas 1)(13 males and 11 females)and 29 patients in the moderate to severe group(Fazekas 2,Fazekas 3)(11 cases,18 women).Meanwhile,22 cases(12 males and 10females)without WML were set as control group.Cognitive function was assessed by the Mini Mental State Scale(MMSE)and the Montreal Cognitive Assessment Scale(MoCA)for all subjects,and relevant statistical methods were used to analyze the characteristics of cognitive impairment associated with WML classification.Results 75 cases were divided into no WML group,WML mild group,and WML moderate and severe group.After the cognitive function score,the total scores of MMSE and MoCA in WML group were significantly lower than those without WML group,and the difference was statistically significant(P <0.05);In the related cognitive domain impairment studies,it was found that visual space and executive ability,attention,speech function,abstract thinking ability,delayed memory and orientation were significantly impaired in the moderate to severe WML group,compared with the group without WML,there was a statistically significant in scores.(P<0.05).Conclusion(1)WML is associated with cognitive impairment,and the more severe the WML,the more obvious the decline in cognitive function.(2)WML can damage multiple cognitive domains,including visual space and executive function,attention,language ability,abstract thinking,delayed memory and orientation,and the moderate and severe WML cognitive domain damage is even worse.
