Objective To retrospectively analyze the clinical manifestations,laboratory examinations,imaging data and endoscopy of adult patients with abdominal allergic purpura,so as to improve the understanding of the disease.Methods The patients with adult allergic purpura were collected from January of 2011 to December of 2017,in General hospital of Ningxia Medical University.All 267 cases were divided by gastrointestinal symptoms,they were divided into abdominal group and non-abdominal group.The two groups of patients were recorded and analyzed by statistical soft to compare the statistical difference between two groups from the causes,first symptoms,clinical symptoms,laboratory tests,imaging examinations,endoscopy and other related indicators,and the clinical features of adult abdominal allergic purpura were summarized.Results 267 adult patients with allergic purpura ranged from 14 to 77 years old,with an average age of 30.04±17.13 years old.The ratio of female to male was 1:1.43,and the highest incidence was in the 14-20 age groups.In the winter and spring,the incidence accounted for 53.2%,46.4% of patients due to infection,foodborne disease.All the patients had rashes during the course of the disease.There was no significant difference in age,sex,onset time,allergic history,joint and kidney injury,hematuria and IgA elevation between abdominal group and non-abdominal group(P> 0.05).The leucocyte count,C-reactive protein increased,serum albumin decreased more significantly,the difference was statistically significant(P <0.05).All patients with abdominal allergic purpura had abdominal pain,combined with gastrointestinal bleeding(73.3%),nausea and vomiting(40.0%),diarrhea(20.0%);Electronic gastrointestinal microscopic findings: mucosal congestion,edema,bleeding spots of different sizes,erosion,ulcers,with duodenal lesions being the most serious.the most serious involvement of the duodenum,ileum lesions.Patients with allergic purpura who started with "abdominal pain" symptoms are easily misdiagnosed,and the rate of misdiagnosis is(25.6%).It is easily misdiagnosed as as hollow organ rupture,hemorrhage,pancreatitis,intestinal obstruction and other diseases.Conclusion 1,Adult allergic purpura is more common in young men,infection and improper diet are the common causes of this disease.2,Compared with the non-abdominal group,the white blood cells and C-reactive protein in the adult abdominal group increased,and the serum albumin decreased more obviously.3,Adult patients with abdominal allergic purpura have severe gastrointestinal symptoms but mild signs.Duodenal involvement is the main cause of gastroscopic mucosal lesions.
