Objective:To analyze the preventive effect of Gushukang Granule on adjacent vertebral body fracture after percutaneous kyphoplasty.Method:The study subjects were diagnosed as single vertebral body(T10-L4)osteoporotic compression fractures(OVCF)from October 2015 to October 2016 in the Department of Orthopaedics,Fuzhou People's Hospital of Fujian University of Traditional Chinese Medicine.Percutaneous kyphoplasty(PKP)patients were enrolled in the study according to the diagnostic criteria of Chinese and Western medicine.A total of 85 patients were randomly divided into two groups:(1)Control group:On the 2nd postoperative day,the basic anti-osteoporosis treatment((calcium acetate capsule)0.6g po qd,Luo Gaiquan(calcitriol capsule)0.25ug po bid),1 month 1 course of treatment,each treatment was suspended for 2days,a total of 6 courses,42 cases were treated,2 cases were shedding,and 40 cases were actually completed;(2)test group:the second day after surgery to the basic anti-osteoporosis treatment,Jiagu Shukang granules 10g after meal boiled water 2 times/On the day,1 month is a course of treatment,and each treatment is suspended for 2 days.According to this method,6 courses are taken,43 cases are treated,3 cases are shedding,and 40 cases are actually completed.A total of 80 patients were completed in the two groups.The patients were 57-84years old,with an average of(67.83±6.52)years old,including 19 males and 61 females.The preoperative and postoperative 1 week,3 months,and 6 months were observed and recorded.VAS score,ODI dysfunction index,bone mineral density before surgery,3 months after surgery,6 months after surgery,and changes in adjacent vertebral height at 6 months after surgery.All patients were followed up for 24 months.The adjacent vertebral body fractures were finally analyzed.Result:1.The gender,age,body mass index(BMI),initial fracture site,vertebral compression degree,bone cement filling volume,and intraoperative cement leakage were compared between the two groups.There was no significant difference(P>0.05).,comparable.2.VAS score:The VAS scores of the two groups after treatment were significantly lower than those before the operation,and the decline of the test group was more obvious with the extension of time.There was no significant difference between the two groups(P>0.05).The VAS scores of the two groups were significantly different between the two groups(P<0.05)..It indicated that the pain of the experimental group was better than that of the control group,and the effect became more and more obvious with the prolongation of treatment time.3.ODI score:The ODI scores of the two groups after treatment were significantly lower than those before the operation,and the decrease of the test group was more obvious with the extension of time.There was no significant difference in the ODI score between the two groups at 1 week after treatment(P>0.05).The ODI scores of the two groups were significantly different between the two groups at the same time(P<0.05)..It is suggested that the improvement of activity dysfunction in the experimental group is better than that of the control group.As the medication course increases,the functional activity recovery is more ideal.4.Bone mineral density levels:The bone mineral density levels of the two groups after treatment were significantly higher than those before surgery.There was no significant difference in bone mineral density between the two groups(P>0.05).There was significant difference in bone mineral density between the two groups(P<0.05).It indicated that the improvement of bone density in the experimental group was better than that in the control group.5.Adjacent vertebral height changes:statistical analysis of adjacent vertebral height changes in the two groups after treatment,using independent sample t test,upper vertebral body P~#,P*,P~?<0.05,statistically significant;The lower vertebral body P~#,P*,P~?<0.05,there was a statistical difference.The difference of adjacent vertebral height between the two groups was significantly different after 6 months.The height compression rate of the adjacent vertebral body in the experimental group was significantly lower than that of the control group.6.Postoperative non-violent fall and adjacent vertebral fractures:After 24 months of follow-up,the number of non-violent falls in the two groups was statistically analyzed?~2=0.139,P~?=0.709(P>0.05),no statistical difference;Compared with the two groups of adjacent vertebral body fractures,?~2=1.829,P~*=0.176(P>0.05),there was no statistical difference.Although there was no statistical difference between the two groups,the non-violent fall rate and adjacent vertebrae in the experimental group The rate of re-fracture was lower than that of the control group.Conclusion:Gushukang Granule can alleviate postoperative pain symptoms,improve quality of life, increase bone density,effectively reduce the loss of adjacent vertebral height,reduce the risk of adjacent vertebral body fracture,and prevent the adjacent vertebral body fracture.It is worthy of clinical promotion.
