Correlation Between EGFR Gene Mutation And Peripheral Blood Inflammatory Markers In NSCLC Patients

Objective:To explore the correlation between the gene mutation status of eidermal growth factor receptor(EGFR)and peripheral blood inflammatory indicators NLR,PLR,LMR and SII in non-small cell lung cancer(NSCLC)patients,and to evaluate the clinical significance and application value of peripheral blood inflammatory indicators in the EGFR gene mutation status of NSCLC patients.Methods:Selected in July 2016 to December 2018 in Shaanxi Province People's Hospital of NSCLC patients.According to the inclusion criteria and exclusion criteria,155 samples were finally collected for lung cancer EGFR gene testing.The clinical data and preoperative blood of all patients were collected.At the same time,NLR,PLR,LMR and SII were calculated according to the definition.The relationship between each indicator and each clinical feature and EGFR mutation status was analyzed.Results:155 NSCLC patients were divided into 92 patients with EGFR wild-type mutation(60 males,32 females)and 63 patients with EGFR mutation(26 males,37 females).1)The EGFR gene status of NSCLC patients was related to the gender,smoking history and pathological type of the patients.The mutation rate of female,non-smokers and adenocarcinoma patients was higher than that of male,smokers,squamous cell carcinoma and other pathological types(P < 0.05).2)NLR showed statistical differences in gender,smoking history and pathological type.NLR of male,smokers and squamous cell carcinoma patients was higher than that of female,non-smokers and adenocarcinoma patients(P < 0.05),In the high NLR group,the proportion of male,smokers,squamous cell carcinoma and low differentiation,patients with EGFR wild type was higher(P < 0.05).In the low NLR group,the proportion of female,non-smokers,adenocarcinoma and highly differentiated,patients with EGFR mutations type was higher(P < 0.05).3)The PLR was statistically difference in different pathological stages.PLR of patients with stage ? was higher than patients with stage ?(P < 0.05).In the high PLR group,the proportion of male patients was higher,on the contrary,in the low PLR group,the proportion of female patients was higher(P < 0.05).4)LMR showed statistical differences in gender,smoking history,pathological stage,EGFR mutation status.LMR of patients with women,non-smokers,? stage,EGFR mutations type was higher than male,smokers,? stage,EGFR wild type(P < 0.05).In the high group,the proportion of the patients with non-senile group,? stage was higher.In the lower groups,the proportion of the patients with elderly patients,?stage was higher(P < 0.05).5)SII in gender,smoking history,pathologic stage,EGFR mutation status differences had statistical significance.The SII in male,smokers,? period,patients with EGFR wild type than women,non-smokers,? stage,patients with EGFR mutations type(P < 0.05).In high SII group,the proportion of smokers,? stage patients was higher,and in low SII group,the proportion of non-smokers,? stage patients was higher(P < 0.05).6)The area under the ROC curve of LMR used to determine EGFR gene mutation was larger than that of NLR and SII,with an area of 0.585.The cut off value was 1.659.The sensitivity was 87.3% and the specificity was 67.4%.Conclusion:1.The EGFR mutation status of NSCLC patients was correlated with NLR,LMR and SII,but not significantly correlated with PLR.2.LMR of NSCLC patients with EGFR mutant type is higher than that of EGFR wild-type,while SII of those with EGFR mutant type is lower than that of EGFR wild-type.Meanwhile,the proportion of NSCLC patients with EGFR wild-type is higher in the high NLR group,and the proportion of NSCLC patients with EGFR mutant type is higher in the low NLR group.3.Compared with NLR and SII,LMR has better diagnostic reference value in the determination of EGFR mutation status.Patients with LMR higher than 1.659 were more likely to have EGFR mutations,and vice versa.
