Objective: This topic intends to study the correlation between carotid atherosclerosis and methionine level in essential hypertension, and explore the early prediction methods for such patients, with a view to providing some guidance for early dietary adjustment and other interventions in patients with essential hypertension carotid atherosclerosis. Methods: According to the Chinese Guidelines for the Prevention and Treatment of Hypertension 2010,149 patients with essential hypertension admitted to Zhongshan Hospital Affiliated to Dalian University from 2017 to 2018 were selected.According to the results of carotid artery ultrasonography,63 patients were divided into simple hypertension group(33males and 30 females, average age 58.87±11.45 years),hypertension with carotid atherosclerosis. There were 86 cases in atherosclerosis group(34 males and 52 females, average age 63.45±8.91 years),and 41 cases in healthy control group(12 males and 29 females, average age 52.56±10.14 years). All patients were examined by carotid ultrasonography, fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-cholesterol),creatinine(Cr),uric acid(UA),alanine aminotransferase(ALT),and aspartate aminotransferase(AST)were measured by fasting venous blood sampling.Methionine(Met)was measured by tandem mass spectrometry. The correlation of methionine level and other indicators with carotid atherosclerosis in patients with hypertension was compared among the three groups. Results: 1.There was no significant difference in total cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol, creatinine, alanine aminotransferase and glutamic oxaloacetic aminotransferase among the three groups(P > 0.05).There was no correlation between the above indexes and carotid atherosclerosis.2.The age, gender, fasting blood glucose, triglyceride and uric acid of hypertension patients with carotid atherosclerosis were significantly different from those of the other two groups, indicating that the above indicators were related to carotid atherosclerosis.3.Methionine levels were measured in hypertensive patients with carotid atherosclerosis(17.94±5.21?mol/L),simple hypertensive patients(14.67±2.46?mol/L)and healthy subjects with(14.11 ±2.48?mol/L).The concentrations of methionine in three groups were compared.Hypertension with carotid atherosclerosis group was larger than simple hypertension group, and larger than healthy group, and P < 0.05 difference was statistically significant, indicating that methionine and carotid atherosclerosis has a certain correlation.4.Logistic regression analysis of the above-mentioned statistically significant indicators of age, gender, fasting blood glucose, triglyceride, uric acid and methionine showed the results:age(P=0.001),gender(P=0.313),fasting Blood glucose(P=0.001),triglyceride(P=0.295)uric acid(P=0.007),methionine(P=0.000),except for the above indicators except triglycerides and gender, all other indicators were statistically significant.Difference,P < 0.05.Methionine was an independent risk factor for carotid atherosclerosis.Age,fasting blood glucose, and uric acid also have a certain effect on the carotid artery in patients with hypertension.5.The area under ROC curve drawn by methionine was 0.703(95% CI 0.626-0.779,P < 0.05),and the optimal critical value was 16.73?mol/L.The sensitivity and specificity of predicting carotid atherosclerosis in hypertensive patients were 57% and 79.8%,respectively.Conclusion: Methionine is an independent risk factor for carotid atherosclerosis.Carotid atherosclerosis is more likely to occur in hypertensive patients with higher methionine concentration. When methionine level(> 16.73?mol/L),the sensitivity and specificity of predicting carotid atherosclerosis in hypertensive patients were 57% and 79.8%.Age,fasting blood sugar and uric acid also had certain effects on carotid atherosclerosis in hypertensive patients.
