Abstract/Summary:
Objective:The traditional treatment for advanced cervical cancer is just radioherapy,but its five-year survival period is less than 60%.In the past two decades,there were so many large-scale prospective randomized researches,which consistently showed thatconcurrent radiotherapy and chemotherapy(CCRT)can significantly reduce the recurrence and metastasis of cervical cancer and effectively improve the prognosis of patients.However,neoadju-vant chemotherapy has a definite effect on reducing the volume of tumor lesions and reducing the tumor burden of patients,thus increasing the possibility of performing radical surgery and radical radiotherapy for cervical cancer;reducing the number of tumor hypoxic cells,improving the sensitivity of cervical cancer to radiotherapy;eliminating subclinical tumor lesions and distant micrometastasis;reducing the positive rate of postoperative margins,clinical staging,thus reducing the risk of rehydration after radical cervical cancer surgery.Therefore,the purpose of this study is to find out whethern-eoadjuvant chemotherapy combined with concurrent chemoradiotherapy in patients with advanced cervical cancer can improve the clinical outcomes and patients’prognosis by retrospective analysis.Method:This study collected clinical data forn 79 patients with advanced cervical cancer at stage IIB to IVB in the 307 Hospital of People’s Liberation Army from January 2012 to January 2016.The patients were diagnosed as cervical cancer by histopathology of cervical biopsy.According to the treatment method,the patients were divided into two groups:observation group with 2 courses paclitaxel+cisplatin regimen and then neoadjuvant chemotherapy of concurrent chemoradiotherapy(NACT+CCRT group)treatment,40 cases;controlling group with concurrent chemoradio-therapy group(CCRT group)treatment,39 cases.In this paper,we conducted the survival analysis and prognostic analysis of these two groups to evaluate the short-term and long-term outcomes of the two treatment methods.Result:The effective rate of NACT+CCRT group was 87.5%,which was higher than CCRT group’s rate of 79.5%,but the difference was not statistically significant(χ~2=0.922;P=0.38).There was a statistically signif-icant difference in OS and PFS survival curves between the two groups within1 year,but no statistically significant difference between survival curves within 2 years and 3 years existed.Within 3 months of concurrent chemor-adiation,the toxic side effects and radiation injury of NACT+CCRT group were significantly relieved.However,after 6 months of concurrent chemora-diation,there were no significant differences in the incidence of toxic side effects and radiation injury between the two groups.Conclusion:Neoadjuvant chemotherapy plus concurrent chemoradi-otherapy has a definite effect on the short-term efficacy of advanced cervical cancerfor patients with advanced cervical squamous cell carcinoma,but the outcome of long-term efficacy is still uncertain.Investigation on larger series with longer follow-up is warranted.
