Objective: To study the safety and reliability of modified banding invagination pancreaticojejunostomy during the perioperative period of pancreaticoduodenectomy through the analysis of postoperative pancreatic fistula and other related complications of patients undergoing this operation method.Methods: In this study,the data from 88 patients who received modified banding invagination pancreaticojejunostomy during the perioperative period of pancreaticoduodenectomy in department of general surgery(VIP ward)of Liaoning cancer hospital &institute from January 2016 to January 2019.These data consisted of the patient's general situation,operation time,postoperative pathological type,postoperative bleeding,postoperative gastric emptying disorder,postoperative drainage volume on 1 d,3 d,5 d,7 d and so on.Postoperative pancreatic fistula diagnostic criteria in this thesis refers to international postoperative pancreatic fistula diagnostic criteria(ISGPF 2005).Results: The average age of the patients was 60.8±8.9(30-79)years old and the operation time was 306.0 ±48.1 min.The pathological type of diseases of the 88 patients were 28 cases of pancreatic head cancer,24 cases of ampullary carcinoma,12 cases of bile duct carcinoma,9 cases of duodenal adenocarcinoma,6 cases of duodenal stromal tumor,4 cases of neuroendocrine tumor,1 case of gallbladder carcinoma and 4 cases of benign disease.The overall incidence of postoperative complications in 88 patients was 23.9%,including 14.8% of grade A pancreatic fistula,2.3% of grade B pancreatic fistula,1.1% of abdominal infection,3.4% of gastric emptying disorder and 2.3% of postoperative bleeding.There was no grade C pancreatic fistula or death during perioperation.Conclusion: Modified banding invagination pancreaticojejunostomy in perioperation of pancreaticoduodenectomy reduced the incidence of postoperative complications such as pancreatic fistula.It is safe,reliable and easy to operate,and has good clinical application value.
