Objective:To observe whether intranasal administration of dexmedetomidine?Dex?has the effects of relieving stress response and stabilizing hemodynamics in patients with radical thyroidectomy,and observing the quality of postoperative recovery and medication adverse reactions,in order to find a better way to apply.Methods:60 patients with ASA I or lI undergoing elective radical thyroidectomy,divided into intranasal Dex group?group IN?,intravenous Dex group?group IV?and the control group?group C?,20 patients were expected in each group.In the group IN,the Dex 0.5?g/kg was administered nasally 10 min before the induction;the group IV was given Dex 0.5?g/kg intravenously for 10 min;the group C was given saline.Observing the mean arterial pressure?MAP?,heart rate?HR?and blood oxygen?SPO2?at the time of patient admission?T0?,intubation?T1?,skin incision?T2?,extubation?T3?and 5 min after extubation?T4?;Epinephrine?E?and norepinephrine?NE?levels were measured in the blood at T0 and T2;Recording patient recovery time and extubation quality score;Observing adverse reactions after drug administration.Results:Compared with the group C,the MAP and HR of the group IN and the group IV were significantly lower at the T2T4 time?P<0.05?;Compared with the group C,the levels of E and NE in the blood of the IN group and the IV group were significantly lower at the T2 time?P<0.05?;The recovery time was not statistically significant in the three groups,compared with the group C,the quality of extubation in the group IN and the group IV was significantly lower?P<0.05?;Compared with the group IV,the times of using atropine in the group IN and the group IV was significantly reduced?P<0.05?.Conclusion:Preoperative intranasal Dex 0.5?g/kg can stabilize hemodynamics during surgery and recovery period,and can reduce the stress response of surgery;Both tntranasal and intravenous administration can improve the quality of patients during recovery and don't affect the patients'recovery time;The incidence of adverse reactions of heart rate slowing is reduced compared with intravenous administration.It provides a reference for the clinical application of Dex.
