Objective: To study the effect of biological valve and mechanical valve replacement on early cardiac function and risks of bleeding after aortic valve replacement in patients aged 50 to 70 years.Evaluate the effect of biological valve and mechanical valve.Methods: Retrospective cohort analysis of patients aged 50 to 70 years who underwent primary isolated aortic valve replacement using bioprosthetic vs mechanical valve in Shengjing Hospital of China Medical University.Randomly selected 35 patients in the group of bioprosthetic and 35 patients in the group of mechanical valve.The left ventricular ejection function,aortic root dimension,left ventricular end-diastolic diameter,left ventricular posterior wall thickness,aortic valve flow velocity,aortic valve transmittal pressure and risks of bleeding were considered as response variables of the valves effect to evaluate different kind of valves.Results: There was no difference in age between two groups.No differences in the left ventricular ejection function(0.36%±11.40% vs 1.17%±10.30%),left ventricular posterior wall thickness(0.13mm±1.58 mm vs 0.13mm±1.73mm)or left ventricular end-diastolic diameter(-3.83mm±7.42 mm vs-5.97±6.66)were observed in patients with mechanical valve compared with bioprosthesis.The difference between,aortic valve flow velocity(1.99m/s±1.01m/s vs 1.23m/s±1.43m/s)and aortic valve transmittal pressure(62.75mmHg±28.40 mmHg vs 49.10mmHg± 23.13mmHg)were observed in patients with mechanical valve compared with bioprosthesis,and the difference are statistically significant 。 However,the cumulative incidence of major bleeding was higher in the mechanical prosthesis group(22.9% vs 5.7).Conclusion: The mechanical prosthesis has an advantage over the bioprosthesis in aortic valve transmittal pressure,aortic valve flow velocity,aortic root dimension.In ejection fraction,left ventricular diastolic diameter and left ventricular posterior wall thickness,the type of artificial valve had little effect on it.Among patients aged 50 to 70 years who underwent aortic valve replacement with bioprosthetic compared with mechanical valve,these findings suggest that bioprosthetic valve may be a reasonable choice in patients aged 50 to 70 years if the patient does not have obvious contraindications.
