Objective: To investigate the clinical features and prognostic factors of patients with pancreatic adenosquamous carcinoma.Methods: Retrospective analysis of clinical data,imaging examination and pathological findings of 32 patients with pancreatic adenosquamous carcinoma who underwent surgical treatment from January 2006 to August 2018 in Shengjing Hospital of China Medical University,followed up,and related factors affecting prognosis Cox risk review analysis.Results: Of the 32 patients with pancreatic adenosquamous carcinoma,22 were male and 10 were female,with an average age of 60.1 years and a median age of onset of 59.5 years.Thirty patients presented with symptoms,including abdominal discomfort,abdominal distension and other abdominal discomfort in 20 cases,abdominal pain with yellow staining in 3 cases,fatigue or weight loss in 2 cases,yellow staining in 3 cases,and back discomfort in 2 cases;2 cases had no obvious symptoms and only physical examination revealed.There were 15 patients with tumors located in the head and neck of the pancreas and 17 tumors at the tail of the pancreas.A total of 30 patients underwent CT examination,of which 28 patients diagnosed pancreatic mass,the other 1 patient diagnosed ampulla,and 1 patient diagnosed with common bile duct.In 15 patients with pancreatic head occupying,CT clearly showed the presence of low biliary obstruction and pancreatic duct dilatation in 13 cases.All patients underwent surgery.All cases were followed up,and 25 patients died.One of the patients was found to have extensive systemic metastases in the first month after surgery and survived for 1.8 months.Six patients had postoperative radiographic data and found distant metastasis in a short period of time.The most common metastatic site was the liver.Seven patients survived until follow-up.The average survival time of this group of patients was 20.41 months,and the median overall survival time was 9 months(95% Cl: 4.45-13.54 months).The 1-,3-,and 5-year survival rates of patients with pancreatic adenosquamous carcinoma were 39.7%,15.4%,and 6.7%,respectively.Conclusion: Pancreatic adenosquamous carcinoma is a relatively rare pancreatic malignancy with early typical clinical manifestations.Pancreatic CT,MRI and endoscopic ultrasound can be helpful in the diagnosis of the disease.The disease has a poor prognosis,and early surgery may prolong the patient's disease-free survival.
