Purpose: Through retrospective analysis of the energy spectrum CT images of patients with bone islands and osteoblastic metastases,combined with multi-parametric imaging features for comprehensive comparative analysis,the purpose of this study was to investigate the clinical application value of energy spectrum CT imaging in the identification of bone islands and osteoblastic metastases.Methods: Collected 46 cases of pathologically confirmed malignant tumors in a patient CT scan of the chest and abdomen gem spectrum,select high density limitations within the vertebral body as the observation target,imaging data combined to make the final diagnosis of metastases to the bone or bone island,The region of interest is placed in each localized high-density region,and the CT and SD values of 50 Kev,70Kev,90 Kev,110Kev,and 130 Kev single-energy images are measured,and the calcium(water)and water(calcium)-based material density images are reconstructed.,measuring the density of the base material density of the lesion.Differences in measurements between the osteogenic metastases and the bone islands were compared using an independent sample t-test.The ROC curve is used to judge the diagnostic performance of each image parameter.Result: The CT value?SD value and K value of each single energy of bone island were all higher than that of osteogenic metastatic tumor(p<0.05).The water(calcium)density of bone island was lower than that of bone metastatic tumor and the calcium(water)density was higher than that of bone metastatic tumor(p<0.05).The ROC curve analysis of energy spectrum CT characterietic parameters on bone island and osteogenic metastatic tumor showed that the single energy 130 Kev image SD value had the best efficacy in identifying bone island and osteogenic metastatic tumor,and the corresponding AUC,sensitivity and specificity were 0.909(95%CI:0.832-0.986),85.7% and 81.8%,respectively.Conclusion: The energy spectrum characteristic parameters of bone island and osteogenic metastases have certain differences.the techniques of energy spectrum CT single energy imaging,energy spectrum curve and material separation can improve the accuracy of diagnosis especially the SD value of 130 Kev single energy image.the identification of the two has a higher diagnostic efficiency.
