Abstract/Summary:
Objective: To analysis the clinical features,the microbiology characteristics and the risk factors associated with the mortality of adult patients with bloodstream infection(BSI)as well as myelodysplastic syndromes(MDS)patients with BSI in Shanghai Jiao Tong university affiliated sixth people’s hospital,to provide the basis of disease evaluation in clinical and rational use of antibiotics.Methods: Clinical materials and microbiology characteristics of adult patients with BSI from January 2011 to March 2016 as well as MDS patients with BSI from January 2011 to August 2016 in Shanghai Jiao Tong university affiliated sixth people’s hospital were retrospectively reviewed and analysed by SPSS statistical package,respectively.Results: A total numbers of 1136 adult patients with BSI from January 2011 to March 2016 were enrolled in this research,158 patients died and the overall mortality was 13.9%;Escherichia coli was the most common pathogen;heart disease,chronic kidney disease stage 3 or worse,cerebrovascular disease,tumor,hemoglobin concentration <90g/L,and ICU therapy were associated with higher mortality in adult patients with BSI by multivariate analysis.A total of 51 MDS patients with BSI from January 2011 to August 2016 were enrolled in this research,12 of patients died and the overall mortality was 23.5%;Escherichia coli was the most common pathogen;hypoalbuminemia and antibiotic monotherapy were associated with higher mortality in MDS patients with BSI by multivariate analysis.Conclusions: The predominant pathogens of BSI in adults and MDS patients with BSI were Gram negative bacteria and the most common pathogen was Escherichia coli.Heart disease,chronic kidney disease stage 3 or worse,cerebrovascular disease,tumor,hemoglobin concentration <90g/L and ICU therapy were associated with higher mortality in adult patients with BSI.Hypoalbuminemia and antibiotic monotherapy were independent risk factors for mortality in MDS patients with BSI.
