The Value Of SST2 In Diagnosing CHD

Objectives:By testing the levels of(soluble growth stimulating express gene 2)sST2 of patients with coronary heart disease,to investigate its clinical value in the diagnosis of coronary heart disease and the prediction of the severity of coronary stenosis.Methods:A total of 179 patients with coronary heart disease admitted in cardiovascular department in Shanghai General Hospital Affiliated to Shanghai Jiaotong University from Aug.2016 to Feb.2017 and 55 healthy examinations were enrolled.All cases are divided into 3 groups,normal control group(55 cases),patients with chest pain or chest distress but having negative PTCA examine result(PTCA normal)group(52 cases),coronary heart disease(CHD)group(127 cases).CHD group is divided into 4 groups: SAP group(34 cases),UAP group(33 cases),NSTEMI group(26 cases),STEMI group(34 cases).Serum sST2 was tested by ELISA test at the second day after patients being hospitalized.Record the PTCA results of the patients and calculate their Gensini scores.Analyze the difference of sST2 levels between different groups,and the correlation between sST2 level and Gensini score in CHD group.Obtaining the fitting curve of sST2 for diagnosing CHD.Result:1.The serum sST2 level of CHD group?PTCA normal group and normal control group are 28.24(IQR 20.79-59.26)ng/ml,23.82(IQR 18.56-28.60)ng/ml,16.88(IQR 14.87-22.79)ng/ml.The serum sST2 level of CHD group is significantly higher than PTCA normal group and normal control group(P < 0.001);The serum sST2 level of PTCA normal group is significantly higher than normal control group(P<0.001).2.The serum sST2 level of CHD group?PTCA normal group and normal control group are 22.14(IQR 17.97-26.80)ng/ml?26.70(IQR 19.64-31.45)ng/ml?28.10(IQR 20.36-65.51)ng/ml?70.80(IQR 49.47-92.25)ng/ml.The serum sST2 level of UAP group and SAP group both have no statistical difference with PTCA normal group(P=0.121?0.584,P>0.05).The STEMI group has the hightest serum sST2 level and the NSTEMI group has the second-hightest serum sST2 level,they both have statistical difference with PTCA normal group(P=0.013?0.00,P<0.05).3.The serum sST2 level of CHD group has significantly correlation with Gensini score,P<0.001,r=0.474.4.In ROC analysis,area under the curve(AUC)of sST2 in diagnosing CHD is 0.742(95%CI=0.680-0.804,P<0.001).The cut-off value of sST2 is 25.99ng/ml,with the sensitivity of 62.2%,specificity of 75.7%.Conclusions:The serum sST2 level of CHD group has significant correlation with the severity of coronary stenosis.The serum sST2 level can be used as the predictor of coronary heart disease.
