The Effect Of Chen Shi Jiang Zhuo Decoction On Patients With Diabetic Kidney Disease (Stage CKD3a) Of Spleen-kidney Qi Deficiency With Stasis And Its Effect On Urinary CTGF And Serum HGF

ObjectiveTo observation the clinical effect of Chen Shi Jiang Zhuo Decoction on patients with diabetic kidney disease(stage CKD3a)of spleen-kidney qi deficiency with stasis,as well as the changes in the levels of urine CTGF and serum HGF.To explore the mechanism of Chen Shi Jiang Zhuo Decoction for reducing turbidity in delaying the progression of renal function injury in patients with diabetic kidney disease(stage CKD3a).Methods64 patients who were diagnosed with diabetic kidney disease(stage CKD3a)of spleen-kidney qi deficiency with stasis at the People's Hospital of Fujian province from January,2018 to January,2019 were selected,and a simple randomized method was used to divide them into the group A and the group B,with 32 cases in each group.Group A was treated with simple western medicine,and group B was treated with Chen Shi Jiang Zhuo Decoction treatment on the basis of group A,the course of treatment was 60 days.To observe eGFR,Scr,24 hours urinary protein quantification,blood lipids,blood glucose,serum HGF,urine CTGF,and TCM syndrome integral changes were compared before and after treatment,and correlation statistical analysis was performed.Results1.The two groups of patients before treatment,such as general clinical data,such as gender,age,eGFR,Scr,24 hours urinary protein quantification,blood lipids,blood glucose,urine CTGF,serum HGF and TCM syndrome scores were not statistically significant differences(P>0.05);2.The levels of TC,TG,LDL-C,and 24 hours urinary protein quantification in the two groups after treatment were decreased,Scr decreased,eGFR increased,the difference was statistically significant(P<0.05),and the treatment group was superior to the control group(P<0.05);The levels of FGB,2hPG,HbA1 c in the two groups after treatment were decreased(P<0.05),however,there is not statistically significant difference between the two groups(P> 0.05);3.DKD group urine CTGF,serum HGF level of two groups was obviously higher than that of healthy control group(P=0.000);After treatment,the treatment group and the control groups serum HGF in both groups increased comrared,urine CTGF in both groups decreased compared with that before treatment(P<0.05);and the treatment group was superior to the control group(P <0.05);4.Correlation analysis: After treatment,the treatment group urine CTGF was positively correlated with 24 hours urinary protein quantification,was negatively correlated with eGFR;serum HGF was negatively correlated with 24 hours urinary protein quantification,was positively correlated with eGFR;urine CTGF was negatively correlated with serum HGF(P<0.01);5.The total effective rate of TCM syndromes in the treatment group and the control group was 83.87% and 61.29%,with statistical significance(P <0.05);6.The treatment group and the control group comprehensive clinical efficacy were 87.10% and 64.52%,the difference was statistically significant(P <0.05);Conclusion1.Chen Shi Jiang Zhuo Decoction significantly improves the clinical symptoms of patients with diabetic kidney disease(stage CKD3a)of spleen-kidney qi deficiency with stasis,and the curative effect is better than that of western medicine alone;2.Blood lipids,urine protein,kidney function of two groups were improved after treatment.The treatment group was superior to the control group of curative effect.This indicates that the combination of Chen Shi Jiang Zhuo Decoction was superior in reducing urine albumin,regulating lipid metabolism,and improving renal function of the patients is superior to the control group;3.Compared with the healthy group,urine CTGF,serum HGF levels increased in patients with diabetic kidney disease(stage CKD3a)of spleen-kidney qi defciency with stasis;After treatment,the treatment group urine CTGF was positively correlated with 24 hours urinary protein quantification,was negatively correlated with eGFR;serum HGF was negatively correlated with 24 hours urinary protein quantification,was positively correlated with eGFR;urine CTGF was negatively correlated with serum HGF(P<0.01);It suggested that urine CTGF may reflect the degree of renal damage in patients with DKD;4.Chen Shi Jiang Zhuo Decoction may improve renal tissue metabolism and renal function by increasing serum HGF and lowering urinary CTGF level,and delaying the progression of DKD.
